Objective(s):The purpose of this study was to investigate the distribution of 5-HTTLPR gene polymorphism among train drivers in Kunming Railway Administration.The relationship between blood pressure value of train driver and 5-HTTLPR gene polymorphism,occupational environmental factors and the interaction of 5-HTTLPR gene polymorphism and occupational environmental factors on blood pressure value of train drivers were also discussed.To provide advice and suggestions for train drivers related to hypertension,and to provide scientific basis for the formulation of relevant policies for the prevention and treatment of hypertension among the professional population of train drivers.Methods:A case-control study of 1:1 was conducted in this study.388 employees of Kunming Railway Bureau were selected as subjects.The general demographic characteristics,life behavior habits and general occupational characteristics of the subjects were collected by general questionnaire survey.Pay-return imbalance scale,life stress scale,work-family conflict scale and job burnout scale were used to collect the data of special occupational factors of train drivers,and blood samples were collected for laboratory examination(DNA extraction and DNA gel electrophoresis).Statistical analysis:the basic situation of the subjects(general demography,biological characteristics,life habits),occupational factors,5-HTTLPR gene polymorphism were described.Aand the measurement data were expressed by mean(standard deviation.,t-test was used to compare between groups.The counting data were expressed by coiposition ratio and ?2 test was used to compare the rates between the two groups.Principal component analysis was used to analyze the contribution rate of univariate influencing factors of hypertension.Multivariate non-conditional Logistic regression model was used to analyze the relationship between occupational environmental factors and hypertension.Analysis of the relationship between the number of risk factors affecting hypertension and hypertension by Spearman correlation coefficient.The interaction between 5-HTTLPR gene polymorphism and occupational environmental factors was analyzed by means of 2 × 4 forked health analysis table.Results:1.There was a significant difference in the frequency distribution of 5-HTTLPR genotype between the hypertensive group and the non-hypertensive group(x 2=7.040,P=0.030).The frequency of SS genotype in non-hyperglycemia group(61.34%)was higher than that in hypertension group(48.45%)(?2=6.506,P=0.011).The frequency distribution of 5-HTTLPR allele in hypertension group was significantly higher than that in non-hypertension group(?2=8.835,P=0.003).The allele frequency of S type in non-hypertension group(74.74%)was higher than that in hypertension group(64.95%).The distribution of 5-HTTLPR genotype was significantly different between the two groups under the dominant genetic model(x 2=6.506,P=0.011).The risk of hypertension with LL+SL genotype was 1.668 times higher than that with SS genotype.2.A total of 388 subjects were enrolled in this study,including 194 patients(50.00%)in hypertension group and 194(50.00%)in non-hypertension group.The subjects in both groups were male.The mean age of hypertension group was(41.24±6.98)years,and that of non-hypertension group was(40.82±6.92)years old.The mean working age was(21.48±7.77)years in hypertension group and(21.90±7.78)years in non-hypertension group.There was no significant difference in age,working age and type of work between hypertension and non-hypertension groups.3.Univariate analysis of the relationship between occupational environmental factors and hypertension showed that waist circumference(t=7.514,P<0.001),heart rate(t=3.903,P<0.001),physical exercise(?2=5.211,P=0.022),smoking(?2=9.107,P=0.003),alcohol consumption(?2=4.590,P=0.032),high salt diet(?2=4.131,P=0.042),degree of external effort(?2=18.993,P<0.001),degree of return(?2=11.241,P<0.001),degree of internal input(?2=3.946,P<0.047),Payback ratio(?2=8.868,P=0.003),degree of life stress(?2=5.009,P=0.025),degree of work-family conflict(?2=5.496,P-0.019),degree of job burnout(?2=14.117,P<0.001).There was a significant difference in the distribution between the two groups.4.Principal component analysis showed that the factors affecting hypertension of train drivers were mainly composed of 6 principal components.The first principal component is job burnout,high salt diet,external pay degree.The second principal component is the return ratio,the degree of return,and the degree of internal investment.The third principal component was drinking,smoking,and 5-HTTLPR gene polymorphism.The fourth principal component is waist circumference,physical exercise,smoking.The fifth principal component is high salt diet,external pay degree,pay return ratio,job burnout degree.Sixth principal component heart rate,5-HTTLPR gene polymorphism,physical exercise.5.Multi-factor Logistic regression analysis showed that the regression equation was valid(P<0.001).Waist circumference(P<0.001),heart rate(P=0.017),reward degree(P=0.019),job burnout(P=0.014)and 5-HTTLPR gene polymorphism(P=0.042)entered the equation Y=-10.740+0.091X1+0.024X2-0.670X8+0.877X,3+0.428 X14.Waist circumference(OR=1.095),heart rate(OR=1.025),job burnout degree(OR=2.404)and 5-HTTLPR gene polymorphism(OR=1.534)were risk factors for hypertension.Degree of return(OR=0.512)is a protective factor for hypertension.The high risk of high blood pressure was 2.404 times higher than that of low burnout.The risk of hypertension in the SL+LL genotype of the 5-HTTLPR gene was 1.534 times higher than that of the SS genotype of the 5-HTTLPR gene.The risk of a high rate of return is 0.512 times higher than that of a low return.6.The analysis of interaction between 5-HTTLPR gene polymorphism and occupational environmental factors showed that there was an additive interaction between 5-HTTLPR gene polymorphism and waist circumference(S=1.196,RERI=0.671,AP=0.132).And the results show that there is a positive interaction between the two factors.The results showed that there was an additive interaction between the polymorphism of 5-HTTLPR gene and the degree of reward(S=1.502,RERI=0.720,AP=0.228),and the results showed that there was a positive interaction between the two factors.There was an additive interaction between 5-HTTLPR gene polymorphism and job burnout(S=0.582,RERI=0.995,AP=0.289),and the results showed that there was a positive interaction between the two factors.The results showed that there was multiplicative interaction(ORint=1.094)between 5-HTTLPR gene polymorphism and job burnout.When both 5-HTTLPR gene polymorphism and job burnout degree were risk factors,the risk of hypertension might be increased.7.Spearman correlation coefficient was 0.318(P<0.001)in the correlation analysis between the number of risk factors of hypertension and hypertension of train drivers,and the correlation coefficient was statistically significant,and the correlation coefficient was significantly higher than that of the control group(P<0.05).There is a dose-response relationship between the number of risk factors of hypertension and hypertension of train drivers,that is,the more risk factors of hypertension in train drivers,the more likely they are to suffer from hypertension.Conclusion(s):1.The SL+LL genotype in the 5-HTTLPR gene polymorphism of train drivers increases the risk of hypertension.2.Waist circumference,heart rate,physical exercise,smoking,drinking,high salt diet,pay return ratio,external pay degree,return degree,life pressure level,degree of internal investment,degree of work-family conflict,the degree of job burnout are the influencing factor of train driver hypertension.Principal component analysis showed that occupational factors contributed the most to the development of hypertension,followed by life habits,biological characteristics and 5-HTTLPR gene polymorphism.3.There is an additive interaction between 5-HTTLPR gene and waist circumference,reward degree and job burnout degree of train driver.There is multiplicative interaction between 5-HTTLPR gene and job burnout degree of train drivers.4.There is a dose-response relationship between the number of risk factors of hypertension and hypertension of train drivers,that is,the more risk factors of hypertension in train drivers,the more likely they are to suffer from hypertension.
