Study On The Effect Of Managing Chronic Diabetes Patients Based On A Chronic Disease System Software

Objectives:Using chronic disease system software as a tool to research the differences in management effects between the pharmacist-led "Internet+" and traditional chronic disease management mode from the perspective of pharmacists,and discuss the advantages and shortages of applying chronic disease management system application to community chronic disease management,to verify the value and significance of pharmacists' participation in chronic disease management,and to provide evidence for the innovation and transformation of pharmacists' work.Methods:60 patients with diabetes diagnosed and admitted to the First Affiliated Hospital of Kunming Medical University and the Xiaobanqiao Health Service Center from September 2017 to August 2018 were included.Patients were divided into two groups according to the type of mobile phone used by them and their willingness to use the mobile phone software,the test group(30 cases)adopted the"Internet+" chronic disease management mode,which was mainly used the "CDMS"APP to conduct daily management based on regular outpatient follow-up and regular telephone follow-up;the control group(30 cases)was used the traditional chronic disease management mode and managed only by routine outpatient follow-up and regular telephone follow-up.Follow-up was respectively performed after 1,3,and 6 months intervention,and the following management outcomes were collected:system availability indicator(SUS)of the above software,medication compliance(MMAS-8),fasting blood glucose(FPG),postprandial 2-hour blood glucose(2h PG),the incidence of hypoglycemia,blood pressure(BP),diabetic depression(PAID-20),and quality of life(EQ-5D).Statistical analysis was performed using SPSS 20 software,and P<0.05 was considered statistically significant.Results:There was no significant difference in the basic data between the two groups of diabetic patients(P>0.05).The system availability score for the "CDMS"software is 61.85±9.47 points.The results of medication adherence control analysis showed:the medication adherence of the two groups was significantly increased after 1 month,and the medication adherence of the test group was distinctly higher than that of the control group after 6 months(P<0.05).The results of blood glucose and blood pressure control analysis showed:the FPG of the two groups was improved significantly after 1 month intervention,while the 2h PG was improved obviously after 3 months(P<0.05);the 2h PG in the test group was distinctly lower than that in the control group after 3 months(P<0.05).The SBP of the test group was improved significantly after 1 month,and the SBP of the control group was improved distinctly after 3 months(P<0.05).However,there was no significant improvement in DBP before and after intervention in both groups,and no significant difference in the SBP and DBP between the two groups at 1,3,and 6 months.There were no statistical differences in the rest of the comparisons.The results of diabetes depression control analysis showed:the diabetic depression status of the two groups was improved significantly after 1 month(P<0.05),but there was no significant difference in the improvement of diabetic depression between the two groups at 1,3,and 6 months.The results of quality of life control analysis showed:the quality of life of the two groups was improved significantly after 1 month,and compared with the control group,the quality of life of the test group was distinctly improved after 3 months(P<0.05).Conclusions:Compared with the traditional chronic disease management model,the pharmacist-led "Internet+" chronic disease management model can improve the medication compliance more effectively,control blood sugar more strictly,and achieve a better quality of life of diabetic patients,but the promoting role for the improvement of the incidence of hypoglycemia,blood pressure and diabetes depression need further verification.Using chronic disease system software as a tool to manage diabetic patients,pharmacist can provide pharmaceutical monitoring,such as drug reorganization,drug supervision,adverse drug reaction monitoring,and drug efficacy evaluation,which not only helps the pharmacist to fully play its value,but also better improve the self-management effect of patients with chronic diseases.Therefore,such practices of pharmacists is worth popularizing.
