Effects Of Smartphone-based Health Management Platform On Medication Compliance Of Patients With Chronic Kidney Disease And Hypertension

Objective: The smart phone health management platform was used to conduct remote management and education for patients with chronic kidney complicated with hypertension,observe the changes of medication compliance and blood pressure of patients before and after intervention,and explore the influence of smart phone health management platform on the clinical results of patients.Methods: From January 2018 to October 2018,patients who were clearly diagnosed as chronic kidney disease with hypertension in the Affiliated Hospital of Qinghai University and who were willing to join this study were collected.Remote medication management,blood pressure monitoring,and remote education about chronic kidney disease and hypertension were conducted through the smart phone health management APP.The changes of patients' medication compliance,blood pressure control,knowledge of renal disease and self-efficacy before and after intervention by APP were compared.Results: 1.After 3 months of remote management and education for patients with chronic kidney complicated with hypertension using smart phone health management platform(APP),their medication compliance was improved compared with that before the intervention,the scores before and after the intervention were(4.00 vs 2.00)and the difference was statistically significant(p<0.05).Medication compliance was independent of patients' gender,age,income,education background and number of oral antihypertensive drugs(p>0.05).2.Systolic blood pressure and diastolic blood pressure of the patients after 3 months of remote management and propaganda decreased by 6mmHg and 3.36 mmhg respectively compared with that before intervention,but the difference was not statistically significant(p>0.05).The mean arterial pressure was 7.63 mmhg lower than that before the intervention,and the mean arterial pressure of the patients before and after the intervention was(110.25 vs 102.62)mmHg,respectively,with statistical difference(p<0.05).3.The knowledge of renal disease health was improved after 3 months of remote management and education,and the scores before and after intervention were(19.00 vs 24.00),with statistical difference(p<0.05).Although self-efficacy evaluation was improved compared with that before intervention,there was no statistical difference(p>0.05).Conclusions: 1.The application of smart phone health management platform(APP)for remote management and education of patients with chronic kidney disease and hypertension can improve their medication compliance and blood pressure,as well as improve their knowledge of renal disease health.2.Smart phone health management platform(APP)can be used as an effective intervention for patients with chronic diseases to conduct self-management.
