ObjectiveThrough the exploration of the management model of hypertensive chronic disease in the medical association environment of Changshu City,the effect of the management mode of hypertensive chronic disease in the medical association was studied,and the model was summarized and promoted to achieve two-way referral,graded diagnosis and treatment,and high The awareness of blood pressure patients,to explore the self-efficacy of patients with hypertension,to provide a scientific basis for improving the quality of life of patients with hypertension,improving the accessibility of medical services,and achieving fairness in public medical resources.Subjects and methodsThe subjects were diagnosed as hypertension patients who were diagnosed with hypertension at the age of 35 years old from June 2017 to December 2017 in the First People's Hospital of Changshu City.After the baseline questionnaire data was collected,the whole group was randomly sampled.120 hypertensive patients who met the inclusion and exclusion criteria were randomly divided into management group and control group,with 60 cases in each group.The management group implemented a 6-month chronic disease management intervention.The control group received routine management.After 6 months,the management group and the control group were again investigated for hypertension.The questionnaire included general information,hypertension related knowledge,hypertension related knowledge awareness table,hypertension self-efficacy energy scale,hypertension quality of life scale,and economic related indicators.Pre-designed problems related to the management of hypertensive chronic disease under the medical association of the First People's Hospital of Changshu City,and then organized the managers of the First People's Hospital of Changshu City and the primary medical institutions,and the experts of hypertension in the lead hospital of the Medical Association.Intensive interviews were conducted by hypertensive physicians in primary care institutions.Statistical analysis was performed using SPSS 16.0 software.The measurement data were expressed by x±s.The t test was used for comparison.The count data was expressed by rate.The comparison between groups was ?2 tested.The difference was statistically significant at P<0.05.ResultsAfter the implementation of management,the management group was better than the pre-and post-administration blood pressure values,hypertension related knowledge,self-efficacy,quality of life,monthly average medical expenditure,and average visit time.? The systolic blood pressure and diastolic blood pressure of the management group were significantly lower than those before management.The difference of systolic blood pressure and diastolic blood pressure in the management group was significantly lower than that of the control group,the difference was statistically significant(P<0.05);? The scores of hypertension elated knowledge were higher than those before management,and the scores of hypertension related knowledge scores in the management group were significantly lower than those in the control group,the difference was statistically significant(P<0.05);? The management group managed daily self-efficacy in hypertension.The life dimension score,the health behavior dimension score,the medication situation dimension score,the compliance behavior dimension score,and the self-efficiency total score were significantly higher than those before management.The management group dimensions and total scores were significantly higher than the control group,and the difference was statistically significant.(P<0.05);?The quality of life scores,psychological function quality of life scores,social function quality of life scores,hypertension-specific quality of life scores,and total quality of life in the hypertension quality of life table after management Before the management,the differences in the dimensions and total scores of the management group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05);? After the management group management,the monthly average medical expenditure and the average visit time were significantly lower than before the management.The monthly average medical expenditure and the average visit time of the management group were significantly lower than the control group,the difference was statistically significant(P<0.05).ConclusionIn this study,the management model of hypertensive chronic disease under the medical association has improved the overall life of hypertensive patients,which contributes to the control of hypertension,enhances the awareness of disease,and thus improves the level of self-efficacy.Improving the quality of life of patients and reducing their personal economic expenditures is worth continuing to implement and promote.
