Research background and purpose: the prevalence of obesity is increasing worldwide,according to the present data,by 2030,almost 40% of the population will be overweight and 20% will be obesity.The occurrence of obesity involves a variety of factors,such as lifestyle,genetic factors,neural mechanisms and endocrine,etc.Many studies suggest that the intestinal flora plays an important role in the obesity epidemic,in this study,we aim to explore the role of intestinal flora on the pathogenesis of obesity and related metabolic diseases by detecting the changes of intestinal flora in healthy person,obese patients and obese patients after weight loss surgery.Methods: 8 obese patients were enrolled in the first affiliated hospital of Dalian Medical University from February 2019 to June 2019,including 1 case of male,7cases of female,age ranging from 25 to 57.Fresh feces were collected from healthy person,obese patients and obese patients after weight-loss surgery respectively.By the detecting 16 Sr DNA gene sequencing technology,bacteria spectrum of feces samples were analyzed.Results:1.The general information:Among 8 obese patients,7 cases of female and 1 case of male,with age ranging of 25-57,mean age 37.4±10.9 years old.The general information of 8 cases in healthy group were similar.2.After bariatric surgery,the indexes of fasting venous blood glucose,total cholesterol,triglyceride,blood uric acid,systolic blood pressure and diastolic blood pressure decreased significantly in obese patients(P < 0.05).3.There was significant difference about flora abundance between the healthy group and obesity group(P=0.0124);There was significant difference about the flora abundance between the healthy group and the postoperative group(P=0.0148);There was no significant difference about flora abundance between the obesity group and postoperative group(P = 0.9357).4.There was significant difference about flora diversity between the healthy group and obesity group(P < 0.01);There was significant difference about flora diversity between the healthy group and the postoperative group(P < 0.05);There was no significant difference about flora diversity between the obesity group and postoperative group(P > 0.05).5.There was significant difference about flora structure between the healthy group and obesity group(Anosim:P = 0.009);There was significant difference about flora structure between the healthy group and the postoperative group(Anosim : P =0.0094);There was no significant difference about flora structure between the obesity group and postoperative group(Anosim:P = 0.304).6.By LEf Se analysis of the three groups,There were 8 flora species with significant difference of abundance in three groups,including Acidobacteria,Deltaproteobacteria,Acidobacteriia,Lachnospira,Prevotellaceae,Tannerellaceae,Parabacteroides,Bacteroides-uniformis(P < 0.05).7.In phylum level,the highest proportion of four bacteria in three groups were Firmicutes,Bacteroidetes,Proteobacteria ? Actinobacteria.Compared with obesity group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteria(q = 0.006),Chloroflexi(q = 0.032),Zixibacteria(q = 0.027),Verrucomicrobia(q = 0.003).Compared with the postoperative group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteria(q = 0.002),Chloroflexi(q = 0.002),Zixibacteria(q = 0.04),Verrucomicrobia(q = 0.002);There was no statistical difference between the Obese group and postoperative group.8.At the class level,compared with obesity group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteriia(q = 0.010), Deltaproteobacteria(q = 0.041);Compared with the postoperative group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteriia(q = 0.004),Deltaproteobacteria(q = 0.025);There was no statistical difference between the Obese group and postoperative group.9.At the order level,compared with obesity group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteriales(q = 0.004);Compared with the postoperative group,the top 10 abundance intestinal flora with statistical difference in healthy group were Acidobacteriales(q = 0.004);There was no statistical difference between the Obese group and postoperative group.10.At the family level,compared with obesity group,the top 10 abundance intestinal flora with statistical difference in healthy group were Prevotellaceae(q = 0.047),Bacteroidaceae(q = 0.032);There was no statistical difference about the top 10 abundance intestinal flora between the Obese group and postoperative group.There was no statistical difference between the Obese group and postoperative group.11.At genus level,compared with obesity group,the top 15 abundance intestinal flora with statistical difference in healthy group was Lachnospira(q = 0.003);(no different species were found in the top10 relative abundance;Compared with the postoperative group,the top 15 abundance intestinal flora with statistical difference in healthy group was Lachnospira(q = 0.008);There was no difference between the Obese group and postoperative group.12.At species level,compared with obesity group,the top 10 abundance intestinal flora with statistical difference in healthy group was acteroides_ovatus(q = 0.031);There was no difference species between the healthy group and postoperative group;There was no difference between the Obese group and postoperative group.Conclusions:1 ? After bariatric surgery,the indexes of fasting venous blood glucose,total cholesterol,triglyceride,blood uric acid,systolic blood pressure and diastolic blood pressure decreased significantly in obese patients.2 ? Compared with healthy people,the abundance of intestinal flora in obese patients was significantly altered.3 ? Compared with healthy people,the diversity of intestinal flora in obese patients was significantly altered.4 ? Compared with healthy people,the structure of intestinal flora in obese patients was significantly altered.5 ? Compared with healthy people,the intestinal flora of the top 10 relative abundance in obese patients were significantly changed at different levels of phylum,class,order,family and species.6?Compared with obese patients,there were no statistic difference according to the abundance?diversity?structure and relative abundance top 10 of intestinal flora at levels of phylum,class,order,family,genus and species between obese patients and after weight loss surgery.7?Compared with healthy people,the abundance of intestinal flora in patients with weight-loss surgery was significantly altered.8?Compared with healthy people,the diversity of intestinal flora in patients with weight-loss surgery was significantly altered.9?Compared with healthy people,the structure of intestinal flora in patients with weight-loss surgery was significantly altered.10?Compared with healthy people,the top 10 relative abundance of intestinal flora in patients with weight-loss surgery was significantly different at levels of phylum,class and order.11 ? Intestinal flora is involved in the development of obesity and related metabolic diseases.
