Objective This study aims to investigate the incidence of sudden deafness and the levels of various vitamins in serum by analyzing the levels of vitamin A,B1,B2,B6,B9,B12,C,D,and E in the serum of patients with sudden deafness of different types of hearing loss. Methods The research object of this project includes two populations:the experimental group was 336 inpatients diagnosed with sudden deafness in the four wards of the Department of Otorhinolaryngology Head and Neck Surgery of the General Hospital of the People's Liberation Army from January 2019 to February 2020,and the control group For the 156 non-deaf hospitalized patients who underwent a health examination at the Physical Examination Center of the General Hospital of the PLA during the same period.Among them, there were 166 male patients and 170 female patients in the experimental group, aged 10-80 years(mean 44.1 ± 14.4 years),and the course of disease was 2 days to 3 months. All subjects in the experimental group met the diagnostic criteria of the Guidelines for the Diagnosis and Treatment of Sudden Deafness(2015)[1].There were 82 males and 74 females in the control group. The age range was 16-80 years(mean 45.9 ± 9.6 years). All subjects in the control group had no history of hearing loss and had pure ear audiometry on both ears at 0.125-8kHz.The average hearing threshold was<25dB HL.The serum levels of vitamin A,B1,B2,B6,B9,B12,C,D,and E in the experimental and control groups were measured, and statistical analysis was performed using SPSS 19.0 software.P<0.05 represents the results with statistical significance.According to the guidelines,336 deaf patients were divided into four types according to the hearing curve:(1)low-frequency descending type,53 people,23 males and 30 females, with an average age of 43.5±13.3 years;(2)high-frequency descending type,70 people,36 males and 34 females, with an average age of 42.1±14.0 years;(3)flat decline,98,50 males,48 females, with an average age of 47.4±15.5 years;(4)fully deaf,115 There were 57 males and 58 females, with an average age of 42.5±15.2 years.Divided into three groups according to the degree of hearing loss:(1)mild to moderate hearing loss group,116 people,58 males and 58 females, with an average age of 44.0±13.5 years;(2)severe hearing loss group,105 people,There were 56 males and 49 females, with an average age of 45.4±13.5 years;(3)Extreme hearing loss group,115 persons,52 males and 63 females, with an average age of 42.9±16.1 years.Statistical analysis of vitamin levels in deafness groups and control groups. Results The serum vitamin test results showed that:?In the deafness group and the control group, the distribution of VD was different in age and gender, males were higher than females, young adults were higher than middle-aged and elderly people.VA?VC and VB2 distributions had gender differences, and males were higher than Female, but no age difference.VB1,VB6,VB9,VB12 and VE distributions have no age and gender differences.?Young adults(<45 years old)men, women and middle-aged(>45 years old)men and women, the VD levels in the sudden deafness group were 43.96±13.33,36.01 ±8.97,39.52±12.64,and 36.27±11.65 nmol/L.The VD levels in the control group were 51.73±10.83,44.14±9.66,49.10±8.73,45.18±10.50 nmol/L.The level of VD in the sudden deafness group was significantly lower than that in the control group(P<0.05),and severe VD deficiency patients only appeared in the sudden deafness group, the incidence of sudden deafness increased with the severity of VD deficiency(P<0.05).?The levels of VB1 in the deafness group and the control group were 87.93±15.87 and 95.16±14.08 nmol/L,which were significantly lower than those in the control group(P<0.05).?The VE levels in the deafness group and the control group were 11.08±0.48 and 11.25±0.44 ?g/mL,the deafness group was lower than the control Group, the difference was statistically significant(P<0.05).?In the female population, the VA levels in the deafness group and the control group were 0.72±0.41 and 0.83±0.53 ?mol/L,respectively,and the deafness group was lower than the control group(P<0.05)),And the incidence of sudden deafness increased with the severity of VA deficiency(P<0.05).In the male population, the VA levels in the sudden deafness group and the control group were 0.94±0.48 and 0.96±0.40 ?mol/L,respectively.The difference was not statistically significant(P>0.05).?In the male population, the VB2 levels in the sudden deafness group and the control group were 4.62±0.72 and 4.80±0.77 ?g/L,respectively.VB2 levels were 4.51±0.73 and 4.54±0.57 ?g/L,respectively,with no statistical difference(P>0.05).? In the male population, the VC levels in the deafness group and the control group were 37.90±4.01 and 39.28±4.11 ?mol/L,respectively,and the deafness group was lower than the control group(P<0.05).In the female population, the levels of VC in the deafness group and the control group were 36.07±3.19 and 36.79±4.54?mol/L,respectively.There was no statistical difference(P>0.05).? The levels of VB6,VB9,and VB12 in the deafness group were 32.64±5.08?mol/L,19.15±5.39 nmol/L,530.16±80.36 pg/mL.The levels of VB6,VB9,and VB12 in the control group were 31.99±5.22 ?mol/L,19.26?5.31 nmol/L,526.67?71.83 pg/mL.There was no significant difference in VB6,VB9,VB12 levels between the two groups(P>0.05).Conclusion The correlation between vitamin B6,B9,B12 and sudden deafness has not been observed in this subject, but the decrease in serum vitamin A,B1,B2,C,D,E levels may be related to the incidence of sudden deafness.Clinical work China should pay attention to it.The specific mechanism of these vitamins in the development of sudden deafness needs further study.
