| Objective: To observe the clinical efficacy of Jiawei Bazhen Decoction in treating renal anemia of CKD stage 4 patients with deficiency of qi and blood,and evaluate the efficacy and safety of traditional Chinese and western medicine in the treatment of renal anemia.Methods: A total of 60 patients from February 2019 to December 2019 were selected for outpatient or inpatient treatment of the first affiliated hospital of Guangxi University of Traditional Chinese Medicine and were treated for chronic kidney disease CKD4 stage qi and blood deficiency anemia,all patients signed informed consent.Collect the general information of the patient,TCM syndromes,signs,tongue and pulse,and laboratory-related examination information.The study uses a randomized controlled trial design method.The included study cases are divided into a treatment group and a control group,each of which has a treatment course of 2 months.The control group was given conventional basic treatment(recombinant human erythropoietin injection,polysaccharide iron complex capsules,health education,etc.).The treatment group was given traditional Chinese medicine Bazhen Decoction(Jiawei Bazhen Decoction's drug composition is as follows: Astragalus 30g),Angelica20 g,Codonopsis 20 g,Atractylodes 15 g,Poria 12 g,Rehmannia glutinosa15 g,Chuanxiong 12 g,Baiji 15 g,Rhodiola 12 g,Zhigancao 6g).By comparison with the control group,observe the indicators of "JiaweiBazhen Decoction" for patients with CKD4 renal anemia(qi hemoglobin,red blood cell count,hematocrit,serum iron,ferritin,blood creatinine,blood urea nitrogen,The amount of erythropoietin)and the improvement of the total score of TCM symptoms before and after treatment,so as to evaluate the clinical efficacy and safety of this treatment scheme in patients with renal anemia.Results: 1.Comparison of experimental indicators: Comparison of main indicators of Hb,RBC,and HCT between two groups of patients.After treatment,the differences between groups were statistically significant(P<0.01).In terms of improving Hb,RBC,and HCT test indicators The treatment group was significantly better than the control group;in comparison with SI,SF,and secondary indicators of Scr and BUN,after treatment,SI,SF,and Scr and BUN were compared between the two groups.The difference was statistically significant.(P<0.05),the treatment group was better than the control group in improving SI and SF test indicators and reducing Scr and BUN indicators.2.Comparison of erythropoietin dosage in the two groups of patients in the first month and the second month of erythropoietin dosage,the control group of patients in the first and second months of erythropoietin usage did not change,and The second month of the treatment group was significantly lower than the first month;compared with the second month,the amount of erythropoietin in the two groups was significantly lower than that of the control group.Group(P<0.01),which can show that the use of Chinese medicine plus flavor Jiawei Bazhen Decoction can effectively reduce the monthly dose of erythropoietin used by patients.3.Comparison of total scores of TCM symptoms: After treatment,the total scores of TCM symptoms of the two groups were compared between groups.Thedifference was statistically significant(P<0.01).The treatment group had a significant effect in reducing the total score of TCM symptoms.Better than the control group.4.Comparison of the efficacy of TCM syndromes:the treatment group had different degrees of improvement in the symptoms of fatigue,fatigue,shortness of breath,dizziness,lack of appetite,and lightness and thirst(P<0.05),especially in fatigue,dizziness,and mouth In terms of improving the symptoms of light thirst,the difference was more obvious(P<0.01),and the treatment group was better than the control group;while the improvement of the symptoms of white face and lip color,numbness of the limbs,abdominal fullness,and stool insufficiency,By comparison,the difference was not statistically significant(P>0.05).5.There were no obvious adverse reactions in the two groups of patients during the treatment.Comparison of the total efficacy of the two groups of patients,the total efficiency of the treatment group was 93.4%;the total effective rate of the control group was 70.0%.After analysis of the results of the rank sum test,the difference between the two was statistically significant(P<0.01).The overall efficacy of the disease in anemia was significantly better than that of the control group.Conclusion: Modified Bazhen Decoction can reduce the use of erythropoietin,increase the patient's hemoglobin,red blood cell count,hematocrit and serum iron and ferritin levels,reduce blood creatinine and blood urea nitrogen levels,thereby improving renal function in patients with CKD4 stage;It has obvious effects in improving the clinical symptoms of traditional Chinese medicine.There are no adverse reactions during the use of Jiawei Bazhen Decoction,and the clinical application is safe and reliable. |
