Objective:The purpose of this study was to investigate the clinical efficacy of arthroscopic lesion debridement for the treatment of end Achilles tendinopathy, and to evaluate its effectiveness and safety. Methods:To retrospectively analyze the data of patients diagnosed with end Achilles tendinopathy among the inpatients in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from January 2016 to December 2019.Sixty patients with Achilles tendinopathy that met the inclusion criteria of this study were taken as the research object, and were divided into observation group A(arthroscopic lesion clearance treatment plan)and control group B(Guangxi Traditional Chinese Medicine)according to their different treatment plans.Comprehensive conservative treatment plan for orthopaedics in the First Affiliated Hospital of the University);30 cases in group A(22 males and 8 females),with an average age of(32.80±9.82)years and an average course of disease(13.57±6.04)months,including unilateral limb pain 28 cases(16 cases of right foot,12 cases of left foot),2 cases of bilateral pain;30 cases of group B(23 cases of male,7 cases of female),mean age(31.80±8.23)years,mean course(12.13±2.93)months,Among them, there were 27 cases of unilateral limb pain(16 cases of right foot and 11 cases of left foot),and 3 cases of bilateral pain.Age range from 23 to 54 years old;by comparative analysis of the two groups of patients' age, sex, course of disease, length of stay, cost of hospitalization and different time periods(before treatment,2 weeks,1 month,2 months,3 months,6 months)Pain Visual Analogue Scale(VAS),American Foot and Ankle Association Ankle and Hindfoot Scale(AOFAS Ankle-Hindfoot Scale),and the excellent rate and recurrence of the two groups after 6 months of treatment rate. And statistical analysis of the above observation indicators to observe the efficacy of the two treatment options for Achilles tendinopathy. Results:All the patients participating in the study completed their respective treatment plans, and there were no serious Achilles tendon injuries and no deaths.Among them, the age, gender, and disease course of the two groups were compared(P>0.05),and the difference was not statistically significant. The comparison of the VAS and AOFAS scores before treatment in the two groups showed no difference(P> 0.05).The comparison of hospitalization time and hospitalization cost showed a significant difference, and the difference was statistically significant(P<0.00).The comparison of the VAS and AOFAS scores in the observation group A showed that the difference between the preoperative and postoperative time periods was statistically significant(P<0.00);in group A,2 weeks,1 month,The comparison of VAS and AOFAS scores at 2 months,3 months, and 6 months showed that the difference was not statistically significant(P>0.05).The comparison of VAS and AOFAS scores in group B of the control group showed that there was no statistically significant difference between before treatment and 2 weeks after treatment(P> 0.05);the comparison of VAS and AOFAS scores after 2 weeks and 6 months after treatment showed that The difference was statistically significant(P<0.05);the comparison of VAS and AOFAS scores before treatment and 1 month,2 months,3 months, and 6 months after treatment showed that the difference was statistically significant(P<0.05);treatment The comparison of the VAS and AOFAS scores at 1 month,2 months, and 3 months showed no significant difference(P>0.05);the VAS and AOFAS scores at 2 weeks after treatment and 6 months after treatment were different from those after treatment.Comparisons at 1 month,2 months after treatment, and 3 months after treatment showed that the difference was statistically significant(P<0.05).Comparing the relapse rate between group A and group B after 6 months of treatment, the difference was statistically significant(P<0.05).Compared with the excellent rate of group A and group B after 6 months of treatment, the difference was statistically significant(P<0.05).Conclusion:Arthroscopic lesion debridement and comprehensive conservative treatment of Achilles tendinopathy can achieve a certain therapeutic effect and improve the patient's quality of life, but arthroscopic lesion debridement can relieve patients' pain and discomfort faster, and its efficacy and stability Well,it is not easy to relapse in a short period of time.It can reduce the time and cost of repeated hospitalization due to repeated pain, make patients recover faster, return to normal life or work, and avoid the side effects of long-term medication. The effect of comprehensive conservative treatment is slow, the treatment time is long, and the stability of the efficacy is poor. The symptoms are easy to recur.Long-term drug treatment can bring various side effects, which seriously affects the normal work and life of the patient.
