Objective: To observe the effects of platelet-rich plasma(PRP)combined with cyclic adenosine monophosphate(c AMP)on the histomorphology and the expression of IGF-1 and b FGF in rabbits with Achilles tendinopathy,and analyze the combination of PRP and c AMP to promote tendon repair.The possible mechanism may lay a theoretical foundation for the treatment of tendonopathy with PRP and c AMP.Methods:Forty-eight male New Zealand white rabbits were fed adaptively for 1 week and randomly divided into 6 groups,8 in each group.Group A was treated with PRP,group B was treated with c AMP,group C was treated with PRP and c AMP suspension,group D was treated with normal saline,group E was model control,and group F was blank.A,B,C,D,E groups were referenced to a chemical modeling method(500mg/0.2ml of prostaglandin E2 injected approximately 2.0cm from the calcaneal attachment point,4 times in total to establish a rabbit Achilles tendinopathy animal model Group F will not deal with anything.After successful establishment of the animal model,ultrasonic testing was used to confirm and then different methods were used to intervene.Group A was injected with 1 ml of PRP in the peri-implant area and group B was injected with 1 ml of c AMP suspension in the peri-implant area of the model group.The C group was injected with 1 ml of a mixture consisting of 0.5 ml PRP and 0.5 ml c AMP suspension.The D group was injected with 1 ml normal saline at the model building site,which was once three-weeks for a total of 2 times;E Group and group F will not interfere.Three weeks after the completion of the intervention,the animals were sacrificed and Achilles tendon specimens were collected.(1)The collected Achilles tendon tissue sections were stained and observed by ordinary light microscope to observe the arrangement of Achilles tendon tissue fibers,inflammatory cell infiltration,and cell arrangement.(2)ELISA was used to detect the expression of IGF-1 and b FGF in tendon tissue of each group.Results:(1)Histomorphological observation: Under light microscope,collagen fibers in group C were thicker,arranged more neatly,and the direction of walking was consistent with uniform cell distribution;the thickness of fibers in groups A and B was between the group C and group E,showing waves Changes in shape,arranged slightly messy,visible part of the collagen fibers were longitudinal tearing,the matrix slightly reduced compared with the normal,cell distribution is not uniform,was ribbon;D,Ecollagen fibers are smaller,more serious damage,arrangement Disorganized,sparse and inconsistent in the direction of travel,collagen fibers showed longitudinal tearing,matrix reduction,uneven cell distribution.It is shown that PRP,c AMP and their combination have certain effects on the repair of Achilles tendinopathy,of which the best combination effect is.(2)Determination of IGF-1 expression: Results of IGF-1 assay: From high to low,they were group C(4.861±0.264 ng/ml),group B(2.492±0.061 ng/ml),and group A(1.379±0.031 ng/ml),E group(0.345±0.031 ng/ml),D group(0.344 ± 0.040 ng/ml),and F group(0.296±0.041 ng/ml),the difference between the 6 groups was statistically significant(P <0.001).The difference between group A and group B was significantly higher than that of group E(P<0.05);the difference between group C and group E was statistically significant(P<0.05);the difference between group A and group B was cosmpared with that of group C.Statistical significance(P<0.05);There was no significant difference between D group and F group(P>0.05).There was no significant difference between E group and F group(P>0.05).(3)Measurement results of b FGF expression: The results of b FGF determination were highest in group C(255.460±1.661 ng/ml),group B(125.189±1.875 ng/ml),and group A(63.067±4.337 ng/ml).Ml),D group(33.686±22.555 ng/ml),F group(20.980±0.858 ng/ml),E group(18.838 ± 1.323 ng/ml),the difference between the six groups was statistically significant(P<0.001).The difference between group A and group B was significantly higher than that of group E(P<0.05);the difference between group C and group E was statistically significant(P<0.05);the difference between group A and group B was compared with that of group C.Statistical significance(P<0.05);There was no significant difference between D group and F group(P>0.05).There was no significant difference between E group and F group(P>0.05).Conclusion: PRP contains growth factors that promote tendon repair.c AMP can increase the level of intracellular phosphorylation.The combination of both can increase the expression levels of IGF-1 and b FGF in tendon cells,reduce the infiltration of inflammatory cells around the tendon,and promote tendons.Cell proliferation and differentiation,synergistically promote the repair of tendonopathy.
