Background: On a global scale,Cardiovascular disease(CVD)has become the biggest health threat of human being.In according to the Chinese cardiovascular disease report 2018,in china right now,there are almost 11 million patients with coronary heart disease(CHD).Hence,the early discovery,diagnosis and treatment are crying need,due to the heavy economic burden which was created by such a large population of patients with CHD.The Syntax score is used to quantify the severity of lesion in patients with CHD,during the operation of coronary angiography.however,the discovery,diagnosis and treatment of so many patients those who unable to undergo coronary angiography in time were delayed,due to the limitation of medical facility in the local hospital and so on.Therefore,patients like that in china lead to a serious economic burdens to families and the whole country.mi R-34 a and MHR are playing a more and more significant role in CHD.miR-34 a would lead to the arterial dysfunction,by regulate the target gene to lead the dendritic cells migrating into atherosclerotic plaques and promoting theinflammation in the patients with CHD.MHR(Monocyte/High-density Lipoprotein Cholesterol),an useful assessment of the inflammation in coronary atherosclerosis,is positive correlating with serum CRP level.In a word,both the miR-34 a and the MHR may be used as an inexpensive and convenient parameter to predict the CHD.Purpose: The aim of this study was to investigate the correlationship between the severity of CHD(Syntax score)and the MHR and the expression of mi R-34 a,and to figure out whether the MHR and the expression of mi R-34 a could be determined as a parameter to predict the CHD.Objective and Methods: There are 123 patients who were diagnosed as acute myocardial infarction or stable angina pectoris or unstable angina pectoris and were showed the stenosis of at least one of the artery(such as the right coronary artery,the left main coronary artery,the left cricumflex coronary artery,the left anterior descending branch)is ?50% after underwent coronary angiography were included in our study.Then,the objective were divided into three groups as high SYNTAX score group(?32),middle SYNTAX score group(22~32)and low SYNTAX score group(?22).The clinical indexes of each group were collected,and MHR was calculated according to the monocyte count /high density lipoprotein cholesterol,and the expression of MicroRNAwas calculated by 2-??CT.Finally,Statistical analyses were performed by SPSS version 23.0.Results:1.results of clinical data.1.1 SYNTAX score(P <0.001)?MHR(P <0.001)?miR-34a(P <0.001)?M(P <0.001)? HDL-C(P <0.001)? CRP(P <0.001)were considered statistically significant.1.2 The multiple comparisons of SYNTAX score,MHR,miR-34 a,M,HDL-C in the three groups were showed statistically significant differences(P <0.001)with a increasing tendency.However,HDL-C was in a decreasing tendency.2.results of correlation.2.1 There was a significant correlation(P <0.001)between SYNTAX score and miR-34 a,MHR,M and HDL-C.The SYNTAX score was positively correlated with mi R-34a(r =0.792),MHR(r =0.690),M(r=0.039),CRP(r =0.187,P =0.039).whereas,it was negative correlated with HDL-C(r =-0.35).2.2 There was a significant correlation(P <0.001)between miR-34 a and SYNTAX score,MHR,CRP and M.The miR-34 a was positively correlated with SYNTAX score(r =0.792),MHR(r =0.796),and M(r=0.803),CRP(r =0.317).Whereas,it was negatively correlated with HDL-C(r=–0.346).2.3 There was a significant correlation(P <0.001)between MHR and SYNTAX score,miR-34 a,M,HDL-C and CRP.The MHR was positive correlated with SYNTAX score(r =0.690),miR-34a(r =0.796),M(r=0.748),and CRP(r =0.487).Whereas,it was negatively correlated with HDL-C(r =-0.651).3.results of logistics regression.3.1 MHR,mi R-34 a,M,PLT,HDL-C and age(57-64 Y)were showed statistically significant(P ?0.001)in logistics regression analysis.3.2 All the ‘?' of MHR(?=11.651,95%CI:0.790~22.512),M(?=13.451,95%CI : 1.325~25.577),mi R-34a(?=16.387,95%CI : 2.170~30.603),PLT(?=0.025,95%CI:0.009~0.041)were positive numbers.Whereas,the ‘?' of HDL-C(?=-9.463,95%CI :-18.09~0.832),age(57-64Y)(?=0.026,95%CI:-4.359~0.092)was a negative number.Conclusion: Both MHR and mi R-34 a were positively correlated with Syntax Score and all of them were the independent risk factors of highest Syntax Score,in which may be used to predict the severity of CHD.
