ObjectiveThis study aimed to investigate the current status of readiness for hospital discharge, discharge guidance quality and coping style of patients with lymphoma chemotherapy, and explore factors influencing readiness for hospital discharge of patients with lymphoma chemotherapy as well as the association among readiness for hospital, discharge guidance quality and coping style;and to evaluate the intervention effect of discharge plan on patients with lymphoma chemotherapy,in the hope of providing reference to improve the degree of readiness for hospital discharge and the life quality of patients.Methods(1)From January 2017 to December 2018,using convenience sampling,a questionnaire survey was conducted in 196 lymphoma chemotherapy patients who met the standard in two top-level hospitals in Hengyang city. The questionnaire including the general data, the readiness for hospital discharge scale, the discharge guidance quality scale and the simple coping style scale, was used to analyze the factors of the readiness for hospital discharge of patients with lymphoma chemotherapy and its correlation with the quality of discharge guidance and coping style.(2)From January 2019 to December 2019,patients with lymphoma chemotherapy in the department of hematology in a top-levele hospital in Hengyang city were selected as the research objects, and the included standard lymphoma chemotherapy patients were randomly assigned to the intervention group and the control group according to the sequence of the admission of the patients.Conventional ways of health education was used in control group patients, and discharge planning on the basis of traditional health education intervention was used in intervention group patients. Before the intervention,1 month and 3 months after the intervention, readiness for hospital discharge scale, discharge guidance quality scale, simple coping style scale, lymphoma patients quality of life scale were used in two groups of patients to evaluate the intervention effect.(3)Establish a database and input the data after checking by two people.SPSS18.0 software was used for statistical analysis.Frequency, percentage, mean, standard deviation, analysis of variance,chi-square test were used to describe the data, and statistical methods such as independent sample t test, linear correlation, multiple linear regression analysis, and repeated measurement anova were used for statistical analysis of the collected data. The bilateral test level was set ? =0.05,and the difference of P < 0.05 was statistically significant.Result(1)The score range of readiness for hospital discharge of patients with lymphoma chemotherapy was 70-110,and the average score of readiness for hospital discharge was 90.83±8.40.The standardized scores of individual status dimension, adaptability dimension and prospective support dimension were 21.22±2.95,36.88±3.34 and 32.81±3.14,respectively.The highest score of dimension was expected support, followed by adaptability and personal status.Age,degree of education, type of complications and caregiver are the independent factors of the readiness for hospital discharge of patients with lymphoma chemotherapy.(2)The score range of the discharge guidance quality of patients with lymphoma chemotherapy was 86-157,and the average score of the discharge guidance quality was 128.30±13.81.The score of the required content dimension, the obtained content dimension, and the discharge guidance skills and effect dimension were 29.15±6.82,32.87±7.41 and 95.43±8.02,respectively.(3)The score range of the coping style of patients with lymphoma chemotherapy was 29-48,and the average score of the coping style was 37.44±3.74,of which the score of the positive coping style was 23.28±5.02 and the score of the negative coping style was 14.13±4.32.These results showed that the score of the positive coping style was higher than that of the negative coping style, suggesting that the patients mainly adopted the positive coping style.(4)Pearson correlation analysis was used to analyze the correlation among readiness for hospital discharge, discharge guidance quality and coping style of patients with lymphoma chemotherapy. The results showed that the readiness for hospital discharge was positively correlated with the total and each dimension of discharge instruction quality, the readiness for hospital discharge was positively correlated with the positive coping style(P < 0.01),and negatively correlated with the negative coping style(P<0.01).(5)Before the intervention, there was no statistically significant difference between the two groups in general information, readiness for hospital discharge, quality of discharge guidance, coping style, total score of quality of life and scores in each dimension(P > 0.05).After intervention with the discharge plan for one month and three months, the patients in the intervention group had significantly improved in the total score and each dimension score of readiness for hospital discharge, discharge guidance quality, positive coping style and life quality, and were significantly higher than those in the control group, with statistically significant results(P<0.01).(6)After 1 month and 3 months of intervention, the two groups had higher total score and each dimension score of readiness for hospital discharge, quality of discharge guidance, positive coping style and quality of life than those before the intervention, with statistically significant differences(P<0.01).ConclusionThe readiness for hospital discharge of patients with lymphoma chemotherapy is at a medium level and needs to be improved, which is related to the quality of discharge guidance and positive coping style.Age,degree of education, type of complications and the presence or absence of caregivers are independent factors affecting the readiness for hospital discharge. The intervention of discharge plan can improve the readiness for hospital discharge, optimize the quality of discharge guidance, improve the coping style, relieve the bad mood and improve the quality of life of patients with lymphoma chemotherapy.
