Objective: To observe the clinical effect of Tongyang Lishui foot bath prescription combined with internal medicine on ascites due to decompensated liver cirrhosis with spleen and kidney yang deficiency,and to evaluate its safety.Methods:Retrospectively collected 86 patients who were diagnosed with decompensated ascites with lower extremity edema in liver cirrhosis from May 2017 to December 2019 in the Department of Hepatology,Ruikang Hospital,Guangxi University of Traditional Chinese Medicine.There were44 cases in the control group and 42 cases in the experimental group.The general treatment was the same.The control group was given Western medicine treatment alone,and the test group was added with Tongyang Lishui Fumigation and washing once a day for a week.Observe the changes of TCM symptom scores,liver function,degree of lower extremity edema,electrolytes,body weight,and 24-hour urine output before and after treatment in the experimental group and the control group to evaluate the clinical efficacy.SPSS20.0 statistical software was used for statistical processing,and different statistical methods were selected according to different experimental indicators and data,such as chi-square test,t test and rank sum test.Results:(1)TCM symptom score: both groups can improve the clinical symptoms of patients with abdominal fullness,loose stools,and cold chills(P<0.05),and the improvement effect of the test group is better than that of thecontrol group(P <0.05).However,the improvement of symptoms of Chinese medicine in the two groups of patients with bleak face and lack of appetite was not obvious(P> 0.05).The total effective rate of TCM symptoms:92.85% in the test group and 81.81% in the control group,the difference is significant and has statistical significance(P <0.05).(2)Both groups can reduce the ALT,AST,TBIL levels of patients,ALB increased significantly,but the experimental group compared with the control group,the difference is not obvious,no statistical significance(P> 0.05).(3)Electrolyte:The difference between the two groups before and after treatment is not obvious,and there is no statistical significance(P> 0.05).(4)Double lower extremity edema: Both groups can effectively improve the clinical symptoms of patients with lower extremity edema,and the treatment effect of the experimental group is significantly better than that of the control group(P <0.05).(5)Weight and 24-hour urine volume: Both groups of treatment can reduce the patient's weight and increase the patient's 24-hour urine volume.The effect of the test group is significantly better than the control group(P<0.05).(6)Total clinical effective rate: 88.09% in the test group and 72.72% in the control group,the difference is significant and has statistical significance(P <0.05).(7)Safety index monitoring: No abnormal results related to this experiment were found in patients' blood routine,electrocardiogram,renal function before and after treatment.Conclusions:(1)Tongyang Lishui foot bath can effectively improve patients' clinical symptoms of TCM,reduce patients' weight and lower limb edema,increasepatients' 24-hour urine output,and is safe and effective for electrolyte stability.(2)Tongyang Lishui Foot Bath is safe and effective in the treatment of patients with ascites due to spleen and kidney yang deficiency type liver cirrhosis with lower extremity edema,without toxic and side effects.
