Effect Of Yishen Paidu Mixture On EGFR And Blood Potassium In Patients With CKD3b～4 Stage Non-dialysis Spleen And Kidney Qi Deficiency

Objective: To observe the effect of Yishen paidu mixture on e GFR and potassium in CKD3 b ～ 4 stage non-dialysis patients with spleen and kidney qi deficiency.Method: 60 patients who met the inclusion criteria were randomly divided into the control group(30 cases)and the treatment group(30 cases).The control group was treated with high quality low protein diet,control of blood pressure and maintenance of water,electrolyte,acid-base balance and other conventional basic treatment plus Niaoduqing granules.The treatment group was treated with Yishen Paidu Mixture on the basis of conventional basic treatment for a period of 6 months.The changes of e GFR,blood potassium,serum creatinine,and TCM syndrome scores of the two groups before and after treatment were observed.The e GFR and blood potassium of the patients were evaluated as a whole,and the therapeutic effect and safety of Yishen Paidu Mixture on CKD3 b ～ 4 stage non-dialysis spleen-kidney qi deficiency patients were observed.Results:(1)Effect on e GFR: After 6 months of treatment,intra-group comparison: the difference between the treatment group and the treatment group before and after treatment was statistically significant(P<0.05),with the average e GFR before treatment being 36.09±8.749 ml/min and the average e GFR after treatment being 39.5200±11.96685 ml/min;The difference of e GFR in the control group before and after treatment was statistically significant(P<0.05).The average value of e GFR before treatment was 34.7857±7.13550 ml/min,and the average value of e GFR after treatment was 37.76±9.83979 ml/min.Inter-group comparison: there was no significant difference in e GFR between the two groups after treatment(P>0.05).The results showed that e GFR increased after treatment in the treatment group and the control group,and both groups were effective.(2)Effect on blood potassium: After 6 months of treatment,intra-group comparison: there was no significant difference in blood potassium before and after treatment in the treatment group(P>0.05);There was no significant difference in blood potassium before and after treatment in the control group(P>0.05).Inter-group comparison: there was no significant difference in blood potassium between the two groups after treatment(P>0.05).(3)Effect on serum creatinine: After 6 months of treatment,intra-group comparison: the intra-group comparison of serum creatinine before and after treatment in the treatment group showed statistically significant difference(P<0.05),the average value of serum creatinine before treatment was 178.53 ±36.435 umol/l,and the average value of serum creatinine after treatment was 168.43±40.415 umol/l;The difference of serum creatinine before and after treatment in the control group was statistically significant(P<0.05).the average serum creatinine before treatment was 178.50±21.987 umol/l and the average serum creatinine after treatment was 169.67±28.465 umol/l.Inter-group comparison: there was no significant difference in serum creatinine between the two groups after treatment(P>0.05).The results showed that the serum creatinine of the treatment group and the control group decreased after treatment,and both groups were effective.(4)Effect on TCM syndrome points: After 6 months of treatment,intra-group comparison: the difference between TCM syndrome points before and after treatment in the treatment group was statistically significant(P<0.05);The difference of TCM syndrome scores before and after treatment in the control group was statistically significant(P<0.05).Inter-group comparison: after treatment,the difference between the two groups was statistically significant(P<0.05).The results showed that the TCM syndrome scores of the treatment group and the control group decreased after treatment,but the treatment group was significantly lower than the control group.(5)Effect on TCM Syndrome Efficacy: After 6 months of observation,the total effective rate of the treatment group was 73.33%,while that of the control group was 53.33%.After treatment,the difference between the two groups was statistically significant(P<0.05),and the treatment group was superior to the control group.(6)Number of people entering hemodialysis: After 6 months of treatment,no patients in the treatment group and the control group entered hemodialysis.Conclusion:(1)Yishen Paidu Mixture can improve e GFR of CKD3b～4 stage non-dialysis spleen and kidney qi deficiency patients.(2)Yishen Paidu Mixture has no effect on blood potassium of CKD3b～4 stage non-dialysis spleen and kidney qi deficiency patients.(3)Yishen Paidu Mixture can reduce serum creatinine of CKD3b～4 stage non-dialysis spleen and kidney qi deficiency patients.(4)Yishen Paidu Mixture can improve the clinical symptoms of CKD3b～4 stage non-dialysis spleen and kidney qi deficiency patients.
