Objective:The study aims to explore the effectiveness and safety of modified Shenghua decoction on uterine involution after cesarean section by comparing the clinical quantitative score,laboratory indexes,gynecological color ultrasound and the the occurrence of adverse events.Methods:According to the inclusion criteria and exclusion criteria,60 parturients were selected.They were delivered by uterine cesarean section in the inpatient ward of Jiangsu Province Hospital on Integrated of Chinese and Western Medicine from April 2019 to December 2019.By the principle of random control,they were divided into two groups.There were 30 cases in the treatment group(oxytocin plus modified Shenghua decoction)and 30 cases in the control group(oxytocin).Oxytocin was treated by intravenous drip for 3 days,and traditional Chinese medicine decoction for 14 days as a course of treatment after operation.At the end of treatment,the daily descending height of uterus fundus,the amount and duration of lochia,the change of uterine volume,the score of TCM symptoms,lactation and blood coagulation function were observed and compared between the two groups.Result:1.There was no significant difference in the daily uterine decline height between the two groups on the first day and the second day after operation(P>0.05),but there was significant difference between the treatment group and the control group from the third day to the sixth day after operation(P<0.05).2.There was no significant difference in the amount of lochia between the treatment group and the control group on the first day and second day after operation,P>0.05,but from the third day to the sixth day after operation,the difference was significant between the two groups(P<0.05).3.There was significant difference in the duration of postoperative bloody lochia between the two groups(P<0.01),and there was difference in the duration of lochia between the two groups(P<0.05).4.There was significant difference in the sum of uterine three diameters between the two groups on the sixth day after operation(P<0.05).There was no significant difference in the sum of uterine three diameters between the treatment group and the control group 42 days after operation(P>0.05).5.On the sixth day and fifteenth day after operation,there was significant difference in lactation between the two groups(P<0.05).On the forty-second day after operation,there was no significant difference in lactation between the two groups(P>0.05).6.On the fifteenth day after operation,there was a significant difference in the score of TCM symptoms between the two groups(P<0.01).Forty-two days after operation,there was significant difference in TCM symptoms between the two groups(P<0.05).There was significant difference in the efficacy of modified Shenghua decoction in the treatment of different syndrome types(P<0.01).The effect of modified Shenghua decoction in the treatment of qi-deficiency and blood stasis syndrome was compared with that of qi deficiency syndrome,P=0.005<0.01,and P=0.036<0.05 compared with that of blood stasis syndrome,indicating that modified Shenghua decoction can effectively improve the syndrome of qi deficiency and blood stasis syndrome.7.There was no significant difference in leukocyte count between the two groups on the first day and sixth day after operation(P>0.05).There was no significant difference in erythrocyte count between the two groups on the first day after operation(P>0.05),but there was significant differenceon the sixth day after operation(P<0.05).There was no significant difference in the terms of hemoglobinbetween between the two groups on the first day after operation(P>0.05),but on the sixth day after operation,the level of hemoglobin in the treatment group was significantly higher than that in the control group(P<0.01).There was no significant difference in platelet count between the two groups on the first day after operation(P>0.05),but there was significant differenceon the sixth day after operation(P<0.05).There was no significant difference in prothrombin time between the two groups on the first day and sixth day after operation(P>0.05).There was no significant difference in fibrinogen between the two groups on the first day after operation(P>0.05),but there was significant difference on the sixth day after operation(P<0.05).8.There were no adverse reactions and side effects during the treatment of parturients in both groups.Conclusion:The common syndrome after cesarean section is qi-deficiency and blood stasis syndrome.Modified Shenghua decoction combined with oxytocin is superior to oxytocin in promoting uterine involution after cesarean section.Modified Shenghua decoction can accelerate the descending height of uterus,reduce the amount and duration of bloody lochia,promote uterine contraction,improve postpartum general lassitude,short of breath and reluctant to speak,abdominal pain and other TCM symptoms,and at the same time promote lactation and improve the uterine microcirculation.lt can improve the life quality of women,accelerate the process of uterine involution and promote puerperal rehabilitation eventually.Its safety and tolerance are good,so it is worthy of clinical promotion.
