BackgroundOvarian epithelial carcinoma is one of the most common malignant tumors in the female reproductive system.Because the lesions are often located in the deep of the pelvic cavity and the early symptoms are not obvious.More than 70% of the patients were found to be in the advanced stage,with a survival rate of less than 30%.As the matter of fact,the 5-year survival rate of early patients is as high as 90%.Therefore,it is necessary to find an effective diagnosis method in early stage of the disease.In recent years,various kinds of tumor markers have gradually become the efficacy evaluation in the early diagnosis of cancer,the same as ovarian epithelial carcinoma.,the method has been proved valuable as prognosis evaluation index as well.ObjectiveTo investigate the value of HE4 levels combined with serum CA125 in the diagnosis of early epithelial ovarian cancer by effective diagnostic methods,so as to provide valid experimental basis for the early detection of the very cancer.MethodThe sample pool is filled by 3 control groups of epithelial ovarian cancer patients all from China Energy Chemical Group General Hospital,and they are respectively as group A(55 cases of ovarian epithelial carcinoma patients),group B(58 cases of ovarian benign lesions patients),group C(11cases of border oarian cancer patients).All of them is confirmed by postoperative pathological diagnosis which so called the "gold standard”.In this paper,the concentration levels of serum CA125 and HE4 were detected by electrochemical luminescence method.The obtained data of the 3 groups were statistically processed by SPSS 23.0 to explore the value of HE4 combined with serum CA125 in the diagnosis of early ovarian epithelial carcinoma.Result1.The expression levels of CA125 and HE4 in ovarian cancer group were significantly higher than those in ovarian borderline tumor group and benign lesion group,and they were statistically significant.However,the comparison between the ovarian borderline tumor group and the benign ovarian lesion group was not statistically significant.2.There was no significant relationship between preoperative serum CA125 level and the patient's age,tissue type,pathological stage,lymph node metastasis,and menopause(P>0.05).It was related to the histological grade of the patients(P<0.05).3.There was no significant relationship between preoperative serum HE4 and the patient's age,histological grade,lymph node metastasis,and menopausal status(P>0.05).It was related to the type of tissue credit and pathological stage of the patients(P<0.05).4.The correlation between the expression of HE4 and CA125 in ovarian cancer patients is positive through the shown of pearson=0.481,P<0.05?5.The highest sensitivity and accuracy detection is performed by the combination of serum CA125 & HE4,and the HE4 test has showed the highest specificity.The highest Youden index authenticity(sensitivity + specificity-1)and Kappa value consistency(consistency with pathological results)of serum CA125+HE4 combined detection were Youden=0.530 Kappa =0.526,respectively.6.The combined detection of serum CA125+HE4 has drawn the largest area under the ROC curve,with statistical significance(P<0.05).The optimal critical value of CA125 was 144.405,and the optimal critical value of HE4 was 120.66.7.Compare to imaging and pathological examination,HE4 and CA125 is more economical in the terms of lower cost,spend less time in screening in the terms of time,demands the smallest wounds in terms of the degree of damage.ConclusionThe comprehensive advantage of HE4 combined with CA125 as a tumor marker in screening early ovarian epithelial cancer is higher than that of single application of HE4 or CA125,which is worthy of clinical application.
