BackgrandOcular trauma is one of the most important causes of blindness and low vision.A major case survey in developing countries shows that ocular trauma has taken the second place in the cause of blindness.Internationally,it is estimated that there are 500,000 cases of blinding ocular trauma worldwide each year.With the improvement of economic level and the change of life style,every year,there are millions of patients with ocular trauma in the country.If the patients cannot get timely and effective treatment after the injury,it will have a serious impact on the vision.Serious blindness,even blindness,is the main cause of blindness in children and young adults.Some scholars pointed out that caused by ocular trauma and its complications and sequelae will cause serious economic burden to the state,especially relative to the current health care system in our country lag behind developed countries,and large population base,the number of patients with ocular trauma is more,due to trauma and concurrent disease,patients with sequelae of the impact on the national economy is more serious.About 1.8 million blind people worldwide are caused by ocular trauma,so the prevention and treatment of ocular trauma is the focus of clinical research.By analyzing the characteristics of patients with ocular trauma in China,we find that most of the patients are adult males,and the visual impairment caused by ocular trauma will lead to their visual impairment significantly,which will cause a serious burden on their families and society.Therefore,it is of great significance to investigate the epidemiology of causes of ocular trauma.ObjectiveThrough statistical analysis of the injury causes,nature,age,occupation,time of visit,injury sites,injury degree and treatment methods of mechanical ocular trauma patients in our hospital,to explore the best window of treatment,preventive measures and treatment of mechanical ocular trauma,in order to improve visual function and reduce the degree of visual impairment.MethodsThis study used retrospective analysis,selected 413 cases of mechanical ocular trauma who were admitted to our hospital between July 1,2014 and June 30,2018 as the research object.Detailed records and statistical medical records and clinical data were analyzed.The annual distribution characteristics of the number of hospitalized mechanical eye trauma patients in our hospital from 2014 to 2018,the age,sex,distribution of trauma source and occupational distribution of patients with mechanical eye trauma,and the distribution of eye types,types of eye injuries The distribution of comorbidities,the distribution of the first treatment time,the distribution of treatment methods,and the distribution of visual acuity at hospital were analyzed.The risk factors and prognostic factors of mechanical ocular trauma were analyzed.Results(1)There was an overall comparison between 4 groups of age showed significant difference(P< 0.05).Comprehensive assessment: the recovery of older patients may be relatively poor;(2)The overall comparison between the 6 groups of injury cause showed significant differences(P< 0.05).Comprehensive evaluation: the recovery of patients with sharp instrument injuries and explosive injuries may be relatively poor;(3)There was an overall significant difference among the 5 groups of ocular trauma types(P< 0.05).Comprehensive assessment: patients with blunt and penetrating injuries may have relatively poor recovery;(4)There was an overall significant difference in the four groups of the first treatment time(P< 0.05).Comprehensive assessment: the recovery of patients with a longer treatment time(2 days or later)was relatively poor;(5)There were significant differences between the two treatment groups(P< 0.05).Comprehensive evaluation showed that the recovery of non-surgical patients was relatively poor;(6)Multivariate Logistic regression showed that the patient's age(older),the cause of injury(acute instrument injury + explosive injury),the first treatment time after injury(later)and the treatment method(no surgical treatment)were all major adverse factors for the prognosis of vision recovery(P< 0.05,OR> 1).ConclusionsMechanical eye trauma is the main type of eye trauma.The incidence is affected by factors such as environment and month.The incidence of mechanical eye trauma among young and middle-aged laborers engaged in high-risk work is relatively high,mostly blunt and sharp.Age,cause of injury,location,degree of injury,treatment time sooner or later,and treatment plan are related to its prognosis.In the clinic,we should attach great importance to the personalized treatment of mechanical ocular trauma,strengthen the popularization of self-protection for the corresponding population,and minimize the rate of blindness.
