First chapter Clinical data of intracranial aneurysms patientsObjective To summary and analysis the onset age,sex,anatomy area,clinical manifestations,imaging data and treatment in 121 cases intracranial wide-necked aneurysms(IA)patients.To investigate the morbidity age,area,imaging characteristics and treatment in wide-necked IA,to analysis the causes,individual treatment of the natural history and wide-necked IA,to establish a relatively complete data base of wide-necked IA patient for future clinical treatment of patients with IA and provide reference.Methods To collect the clinical data of wide-necked IA patients which were confirmed by imaging detection in neurosurgery of Second Affiliated Hospital of Nanchang University from January 2014 to December 2015.The clinical data including sex ratio and age of onset of clinical symptoms,imaging,endovascular treatment methods,Hunt a Hess gradewere used to classify(I grade,II grade,III level,IV grade and grade V)arterial aneurysm SAH patients.To choose the timing of surgery and prognosis.To understand the anatomical location of wide-necked IA and the relationship between the parent artery and IA.All data were analysized by SPSS13.0software.Results The group of patients with intracranial aneurysms in these 121 cases were consisted of 48 male cases,73 female cases and the average age were 48.15 years,average age in famale were 49.8 yaers and 46.5 years in male.In the 121 wide-necked IA cases,there are 31 bilateral posterior communicating artery aneurysms cases,accounting for 25.62%,22 cases occurred in the bilateral carotid arteries,accounting for 18.18%,and 28 cases located in the anterior communicating artery,accounting for23.14%,cerebral aneurysm cases were 12 in total,accounting for 9.92%,9 aneurysms cases distributed in the bilateral anterior cerebral artery,accounting for 7.44%,19 aneurysms cases located in the vertebral artery intotal,accounting for 15.70%.Hunt Hess grade in admitted patient : Class I 44 cases,class II 29 cases,class III 27 cases,15 cases in grade IV and 6 cases in class V.stent assisted coil embolization and balloon-assisted coil embolization were adopted in the therapy in wide-necked IA patients in thses 121 cases.Conclusion(1)The incidence of intracranial aneurysms in women were slightly higher than men,and the average onset age of is about 48 years old;(2)The incidence of anterior circulation aneurysms in internal carotid artery was significantly higher than vertebral-basilar artery system;(3)The main clinical manifestation was intracranial hemorrhage;(4)Hunt-Hess grade were classifed inti level I and II level.(5)Stent assisted coil embolization were more common used than conventional balloon-assisted coil embolization in the treatment of wide-necked IASecond chapter Clinical analysis of stents and balloon-assisted embolization treatment of intracranial wide-necked technology aneurysmsObjective To summarize the experience of stent balloon assisted treatment to intracranial wide-necked aneurysms,to analyze clinical and radiographic outcomes,surgical complications and postoperative follow-up of these two kinds of method,to investigate the security,effectiveness and pinning effects in intracranial aneurysms,providing a reference for future clinical therapy.Methods Data of carotid artery aneurysms patients cured by endovascular interventional treatment were collected from January 2014 to December 2015 in neurosurgery department of Nanchang University Second Affiliated Hospital.patients were divided into treatment group and balloon assisted therapy group according to endovascular stent assisted treatment method difference,surgical complications(intraoperative aneurysm rupture,thrombosis,device-related complications),intracranial aneurysm embolization compact degree,clinical evaluation at discharge and follow-up results were collected.All data were cellected and SPSS13.0 software was adopted to analyze the overall treatment effects,when P <0.05 when the difference have statistically significant.Results All Patients were successfully placed a stent or balloon,complete occlusion rate of two patients with showed no significant differences(P>0.05).Prognosis rate in study group was good(94.5%),compared with control group(95.9%),results had no significant difference(P> 0.05),the incidence of complications was no significant difference(P> 0.05).Conclusion In the interventional treatment of wide-necked intracranial aneurysms,stent assisted coil embolization and balloon assisted embolization had no significant differences...
