Objective:To observe the clinical effect of acupotomy combined with bone-chilling on knee osteoarthritis in order to find a more effective treatment. Methods According to the inclusion and exclusion criteria,60 KOA patients who visited the orthopedics department of wuhu hospital of traditional Chinese medicine between January 2019 and January 2020 were selected.According to the ratio of 1:1,60 KOA patients were randomly divided into the control group and the treatment group, with each group containing 30 KOA patients. The control group KOA patients were treated with oral celecoxib capsules and glucosamine hydrochloride tablets, celecoxib capsules once a day,0.2g once, glucosamine hydrochloride tablets twice a day,and0.75 g once.Patients in the treatment group were treated with acupotomology combined with bone-chilling surgery once a week for one course of treatment for 4consecutive weeks. After one course of treatment, both groups stopped treatment.VAS score and knee WOMAC score of the two groups before treatment and after a course of treatment,as well as the overall efficacy after a course of treatment(the knee joint of the severely ill patients with bilateral knee KOA was included in the statistics),and the data were analyzed and summarized using reasonable statistical methods.Results1.Analysis of the basic data of the two groups of patients: before treatment, there was no statistically significant difference between the two groups in the course of disease, age and gender distribution,as well as the comparison of VAS score and WOMAC score(P > 0.05),and the two groups were comparable.2.Change analysis of knee WOMAC score: after a course of treatment, the knee WOMAC score of both groups was significantly lower than before treatment(P <0.05).Both treatments had good clinical efficacy in improving the symptoms of knee disease and improving the quality of life of patients. After a course of treatment, the WOMAC score of the knee joint of the treatment group was significantly lower than that of the control group, and the difference between the two groups was significant(P< 0.05).3.VAS score change analysis: VAS score after one course of treatment was significantly lower in both groups than before treatment(P < 0.05),and both treatments had good clinical efficacy in improving patients' pain.Compared with the control group,VAS score in the treatment group was significantly lower after a course of treatment(P < 0.05),and the treatment group was better than the control group in terms of pain improvement.4.Comparison of overall efficacy: after a course of treatment, the total effective rate of the control group was 90%,including 7 patients with clinical control effect,9patients with obvious effect,11 patients with effective effect and 3 patients with ineffective effect. The total effective rate of the treatment group was 100%,and there were 10 patients with clinical control effect,16 patients with obvious effect,4 patients with effective effect, and 0 patients with no effect. The clinical efficacy of the two groups was relatively compared, and the therapeutic effect of the treatment group was better than that of the control group(P < 0.05).Conclusions1.In terms of VAS pain score, the two treatment methods had the effect of significantly reducing the pain symptoms of the patients, but acupotomy combined with bone-cutting had a better effect on pain improvement compared with oral celecoxib combined with glucosamine hydrochloride.2.In terms of WOMAC score of the knee joint, both treatment methods can significantly improve the knee joint symptoms and improve the quality of life of the patients, but compared with the treatment method of acupotomy combined with bone-piercing,the treatment method is more ideal.3.In terms of clinical efficacy, acupotomy combined with bone-chilling therapy and oral celecoxib combined with glucosamine hydrochloride therapy both have significant effects in the treatment of KOA,and the efficacy of acupotomy combined with bone-chilling therapy is better than that of oral celecoxib combined with glucosamine hydrochloride therapy.4.No serious adverse events occurred during the whole treatment in the two groups.5.Acupotomy combined with bone-chilling is a simple, safe, inexpensive and effective treatment. As a more appropriate conservative therapy,it is worthy of clinical promotion and further research.
