Objective: Analyze the occupational stress,sleep quality and hypertension prevalence of oilfield workers,explore the risk factors of hypertension in oilfield workers and establish prediction models to provide scientific basis for the prevention and treatment of hypertension in oil field workers.Methods: In this study,multistage sampling was used to conduct a field survey of 1,600 oil field workers in 6 Karamay oilfield bases and collect medical examination data.The pay-reward imbalance questionnaire(ERI)was used to evaluate occupational stress,the Pittsburgh Sleep Quality Index Scale(PSQI)was used to evaluate sleep quality,and logistic regression was used to analyze hypertension risk factors.Lasso-logistic,random forest,and support vector machine(SVM)predict models and perform model evaluation and comparison.Results: 1)The survey started with 1600 people.At the end of the survey,180 people who failed the questionnaire and did not complete the physical examination were excluded.A total of 1,420 participants completed the survey.A total of 232 oilfield workers with hypertension were detected in this study,with a prevalence rate of 16.34%.2)Occupational stress detection rate of oil field workers is 46.90%.Men,married,smokers,and drinkers have a higher occupational stress detection rate and more work and internal investment(P value <0.05).3)A total of 385 oilfield workers were found to have sleep disorders in this survey,with a detection rate of 27.11%.Except for sleep efficiency and hypnotic drugs,the PSQI scores in all dimensions were higher than the domestic norm(P value <0.05).The shift scores of PSQI in all dimensions were higher than those without shifts,the difference was statistically significant(P value <0.05),and the sleep quality of those with occupational stress was worse.4)Multivariate logistic regression analysis shows that age(≥45 years old group),ethnicity,drinking,family history of hypertension,occupational stress,overweight,obesity,abnormal creatinine,high uric acid,and high blood sugar are risk factors for hypertension.Education Level is a protective factor for hypertension.5)The AUC value of the Lasso-logistic prediction model is 0.752,95% CI(0.706-0.798),the calibration curve fits the standard curve well,and the decision curve shows that the net benefit rate of the Lasso-logistic prediction model is high.The comparison of the three prediction models shows that the SVM model has a maximum AUC value of 0.86(95% CI: 0.824-0.896)and the highest accuracy,sensitivity,specificity,and positive hit rate.Conclusions: 1)Karamay oilfield workers have a higher detection rate of occupational stress and poor sleep quality;2)Lasso-logistic model can be used as a tool for hypertension risk assessment of oilfield workers.The prediction model provides a scientific basis for the prevention and treatment of hypertension in oilfield workers.
