Retrospective Analysis Of Intertan And Dhs In The Treatment Of Intertrochanteric Fractures In The Elderly

Objective:To retrospectively analyze the therapeutic effect of INTERTAN combined with interlock-ing intramedullary nail and DHS compression sliding nail in the treatment of intertroch-anteric fractures in elderly patients.Methods:through the statistical analysis of Tai'an Central Hospital in recent 10 years using INTERTAN compression interlocking intramedu-llary nail and DHS DHS two therapeutic methods in treatment of senile intertrochanteric fracture is 50 cases,the data are divided into two groups,26 cases in INTERTAN group,DHS group of 24 cases.The age distribution of patients was between 60-89 years old,mean age 70+5.5,20 cases of male patients and 30 female patients,23 cases of intertrocha-nteric fracture of the left and right intertrochanteric fractures in 27 cases,of which fall accident cause intertrochanteric fracture in 36 cases,traffic accident trauma caused intertrochanteric fracture in 14 cases,2 cases of patients with lumbar fracture,3 cases of patients with upper limb fracture,1 cases of patients with rib fracture.Patients with underlying diseases,5 cases of patients with cardiovascular disease,8 cases of patients with diabetes,3 cases of patients with urinary diseases,6 cases of patients with respiratory diseases,1 cases of patients with gastric cancer after operation.According to the intertrochanteric fracture of Evans classification,5 cases of Evans type I,8 cases of Evans type,Evans type 18 cases,Evans type in 14 cases,5 cases of Evans type V,these patients were obtained(12-15)months of follow-up,respectively in patients after 4 weeks and 8 weeks,16 weeks,24 weeks and 12 months after hip joint X-ray,understand the degree of healing of femoral intertrochanteric fracture patients by X-ray,and referring to the Harris hip score of hip joint function recovery was evaluated.Results: the operation time of INTERTAN group was 55.45+6.3 minutes,the operation time of DHS group was 80.69+12.35 minutes,P<0.05,combined with intramed-ullary compression interlocking nail in treatment of intertrochanteric fractures of the time needed to be significantly less than the operation of sliding compression goose head nail.The blood loss in ERTAN group were 143.23+13.60 ml,blood loss in DHS group were 280.56+50.55 ml,P=0.05,INTERTAN group of intertrochanteric fractures treated with intraoperative blood loss were significantly less than DHS group I.The length of incision in INTERTAN group was 6.34 + 0.86 cm,the length of incision in DHS group was 11.25+ 4.53 cm,P = 0.05,INTERTAN group in the treatment of intertrochanteric fracture surgery required surgical incision length is relatively small.Through the follow-up and review of hip joint X-ray showed DHS group of intertrochanteric fracture healing time was(3.35+0.63)months,group INTERTAN intertrochanteric fracture healing time was(3.15+ 0.93)months,P>0.05,INTERTAN and DHS in the treatment of intertrochanteric fracture,postoperative fracture healing need there is no significant difference between the.The good rate of group INTERTAN showed the Harris hip score system was 84.61%,DHS group the excellent rate was 66.67% and the failure rate was 8.33% in group DHS,INTERTAN group after surgery in patients with fracture of hip joint function recovery was better than that of DHS group.Conclusion:INTERTAN combined with interlocking intramedullary nail in the treatment of femoral intertrochanteric fractures is significantly better than the DHS pressure sliding goose head nail.
