The Clinical Effect Early Analysis Of PFNA And InterTAN In The Treatment Of A2(AO/OTA) Type Of Intertrochanteric Fractures

Objective: Intertrochanteric fracture is the common disease, we usually choose routine operation therapy.In many fixtures, the PFNA and InterTAN are more and more popular department of orthopedics physicians because of its remarkable clinical curative effect.Through the observation and analysis the difference between clinical indicators of PFNA and InterTAN in the treatment of type A2 femoral intertrochanteric fracture, and provide a basis for the treatment of type A2 femoral intertrochanteric fracture internal fixation choice.Methods: From March,2013 to December,2014, 159 patients admitted to the Department of orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine in traumatic femoral intertrochanteric fracture, the AO classification, 59 cases of patients who met the inclusion criteria according to the different internal fixation methods for grouping.PFNA group of 46 patients, aged 30-97 years old, average 75.06±13.23 years, 16 cases of male, 30 cases of female; 40 cases in group of InterTAN, aged 41-90 years, mean age 75.15±15.17 years old, 14 cases of male, 26 cases of female.By comparing the two kinds of the fixed mode of operation time, intraoperative bleeding volume, postoperative follow-up records of fracture healing time and according to the Harris hip score difference, evaluating the difference between the two fixed mode. The follow-up time was 4-16 months, average 10 months, the collected information was used for statistical analysis by the SPSS21.0 statistical software package.Results: The two groups in operation time, intraoperative bleeding volume, fracture healing time,hospital costs, the difference was statistically significant(P<0.05); the two groups inhospitalization time,incision length and Harris hip function score, the difference between the two groups was statistically significant.Conclusion: 1, PFNA and Inter TAN in the treatment of type A2 femoral intertrochanteric fracture were allcan obtain good clinical curative effect. But compared with the Inter TAN system, PFNA has the advantages of relatively simple operation, short operation time, blood loss in the operation, and InterTAN has the advantages in the anti rotation, auxiliary reset, broken end pressure, reduce the broken endclearance, maintain a competitive end stability; 2,Both of the two methods have advantages or not, so we suggest choice the right type of internal fixation according to the patient’s physique, bone condition and the operation of internal fixation devices familiarity;3,PFNA may happen the main nail end femoral stress fracture, screw extrusion, coxa vara; InterTAN is a new method of internal fixation, the number of the operation is less, the follow-up time is short, so this kind of internal fixation need long term to observe the long-term effect.
