ObjectiveCerebrovascular disease is a class of diseases that occur in the cerebral vessels,as far as it is concerned,it has become one of the most important causes of death in the world.Cerebral infarction is one of the most common types of cerebrovascular disease and it has a serious threat to human survival and the quality of life. However, drugs that are safe and effective and can be widely used in clinical practice in the treatment of cerebral infarction are still very limited. So far,the treatment for acute cerebral infarction that has been proved effective by evidence-based medicine is ultra early thrombolytic therapy. However, owing to that the application of thrombolytic therapy is restricted by a strict time window and there are still more bleeding complications after thrombolytic therapy, the focus of treatment for acute cerebral infarction falls on interfering the process of apoptosis aftercerebral ischemia. As a result, treatment measures in respect of neuroprotection soon became hot spots for research.Among them, the curative effect of butylphthalide is becoming more and more recognized. However, there is still a lack of sufficient evidence in the clinical study of its specific mechanisms of treatment, and the correlation study of the effects of butylphthalide on apoptotic factors is more rare. Therefore, this experiment was designed to analyse the effects of butylphthalide injection on the plasma levels of Bcl-2 and Bax and the neurologic impairment in patients with acute cerebral infarction, and to explore the specific mechanism of butylphthalide.MethodsA total of 80 patients with acute cerebral infarction were selected who met the entry criteria and were admitted to the Department of Neurology of our hospital from December 2016 to December 2017,they were randomly divided into the experimental group and the control group,40 cases in each group. The control group were given routine treatment and the experimental group were given butyphthalide injection on the basis of control group. We used ELISA method to measure the levels of plasma Bcl-2 and Bax before treatment(within 24 hours of onset),on the seventh day of treatment and on the fourteenth day of treatment. And we assessed the NIHSS scores and safety evaluation before treatment,on the seventh and fourteenth day of treatment. At the same time,40 healthy persons were selected as normal control group and we used ELISA method to measure their levels of plasma Bcl-2 and Bax.Finally, all the above results were compared and statistically analyzed.Results1.The NIHSS scores of the patients in the experimental group and the control group were scored beforetreatment and they were similar and there was no statistical difference(p?0.05);After treatment, they were scored again and the scores of NIHSS on the seventh and fourteenth day after treatment in the two groups were both lower than those before treatment, and the NIHSS score in the experimental group decreased more significantly than that in the control group. The difference was statistically significant(p<0.05).2.Before treatment, the blood was drawn from both groups, the plasma levels of Bcl-2 and Bax of the patients in two groups were measured and there was no significant difference between the two groups(p?0.05),and the levels were all higher than those in the normal control group. On the seventh and fourteenth day,we measured the levels of plasma Bcl-2 and Bax of patients in the two groups again, and found that the levels of plasma Bcl-2 in patients of the two groups were significantly higher than those before treatment, and the levels of plasma Bax were significantly decreased compared with those before treatment, and the concentration changes in the experimental group were more obvious than those in the control group and the differences were all statistically significant(p<0.05).ConclusionThe results showed that butylphthalide could improve the neurological deficit symptoms in the acute phase of cerebral ischemia. It can also increase the level of plasma Bcl-2 and decrease the level of plasma Bax,thereby inhibiting the occurrence of apoptosis. This suggests that butylphthalide has a protective effect on the nervous system, and its mechanism may be related to anti apoptosis.
