Objectives:To investigate the diagnostic value of transbronchial lung biopsy(TBLB)under Lung-pro bronchial navigation and rapid on-site evaluation(ROSE)for pulmonary nodules.Methods:112 patients with peripheral pulmonary nodules on chest CT from December 2018 to March 2019 in bronchoscope room of the Third Affiliated Hospital of Kunming Medical University(Yunnan Tumor Hospital)who underwent fibrobronchoscopy and ROSE were analyzed.They were randomly divided into traditional bronchoscope biopsy(C-TBLB)group and Lung-pro guided bronchoscope biopsy(Lung-pro)group,with 56 cases in each group.The diagnostic rate,biopsy times,operation time,complications and pathological coincidence rate of the two groups were compared to evaluate the diagnostic value of Lung-pro guided bronchoscopy biopsy combined with ROSE for peripheral pulmonary nodules.Results:1 There were no significant differences in the general information and clinical information from patients between the three groups(P>0.05).2 The diagnostic rate of C-TBLB for peripheral pulmonary nodules was 25.0%(14/56)and 76.8%(43/56)in Lung-pro group.The malignant diagnosis rate was 24.3%(9/37)in C-TBLB group and 76.7%(33/43)in Lung-pro group.Benign diagnosis rate was 26.3%(5/19)in C-TBLB group and 76.9%(10/13)in Lung-pro group.Lung-pro group was higher than C-TBLB group,the difference was statistically significant(?2=16.366,p<0.01).3 The success rate of biopsy once was 14.3%(8/56)in C-TBLB group and 75.0%(42/56)in Lung-pro group,the difference was statistically significant(=85.120,p<0.01).The success rate of 3 or less biopsies was 44.6%(25/56)in C-TBLB group and 85.7%(48/56)in Lung-pro group.The difference was statistically significant(?2=46.247,p<0.01).4 The operation time of C-TBLB group was 11.85±4.62min,while that of Lung-pro group was 32.34±6.27min,with significant difference(?2=4.367,P=0.037).5 Two groups of patients had different degrees of blood in sputum after operation.C-TBLB group:1 pneumothorax(1/56),Lung-pro group:2 pneumothorax(2/56),the difference was not statistically significant.6 The coincidence rate of biopsy pathology and postoperative pathology in C-TBLB group was 34.6%(9/26);The Lung-pro group was 84.6%(33/39),with significant difference between the two groups(P<0.01).Conclusion:Lung-pro navigation bronchoscopy combined with ROSE for peripheral pulmonary nodules has high diagnostic coincidence rate and one-time success rate,controllable complications,and is worthy of clinical promotion.
