[Objective]The purpose of this study was to explore the application of bronchoscopy-guided bronchoscopic lung biopsy under the guidance of bronchial ultrasound guided sheath(Endobronchial ultrasound-guided transbronchial lung biopsy with a guided sheath(EBUS-GS-TBLB)combined with rapid on-site evaluation(ROSE)technology in the diagnosis of peripheral lung cancer.[Method]One hundred and thirty-six patients with peripheral CT lung cancer who were admitted to the Third Affiliated Hospital of Kunming Medical University(Yunnan Provincial Cancer Hospital)from 2017 to 2019 were randomly divided into CT-guided according to different lung biopsy methods Chest wall lung biopsy group(CT-PNB)68 cases,EBUS-GS-TBLB combined with ROSE group 68 cases,compare the general information of the two biopsy methods,lung cancer diagnosis rate,complications and postoperative pathological consistency rate,explore EBUS-GS-The clinical value of TBLB combined with ROSE technology in the diagnosis of peripheral lung cancer.[Results]1.General InformationNo matter in terms of medical history or general data,there was no significant difference between the two groups of patients(P?0.05).2.Diagnosis rateThe diagnostic rate of CT-PNB for peripheral lung cancer is 67.6%(46/68),69.1%(47/68)in the EBUS-GS-TBLB group;the malignant diagnosis rate is 72.5%(29/40)in the CT-PNB group,The EBUS-GS-TBLB group was 73.3%(33/45);the benign diagnostic rate CT-PNB component was 60.7%(17/28),and the EBUS-GS-TBLB group was 60.9%(14/23).There was no significant difference in the diagnosis rate of lung cancer between the EBUS-GS-TBLB group and the CT-PNB group(P>0.05).3.ComplicationsThree patients in the EBUS-GS-TBLB group had a fever of 4.4%(3/68)and one patient had a pneumothorax of 1.5%(1/68).In the CT-PNB group,9 patients had fever 13.2%(9/68),16 patients with pneumothorax 23.5%(16/68),8 patients with bleeding 11.8%(8/68),and 2 patients with chest pain 2.9%(2/68).There were significant differences in complications between the two groups(?2?10.810,P?0.01).4.Postoperative pathological coincidence rateThe coincidence rate between the biopsy pathology and postoperative pathology in the CT-PNB group was 53.4%(15/28);the EBUS-GS-TBLB group was:84.6%(33/39),the difference between the two groups was significant(?2=7.559,P?0.01).[Conclusion]EBUS-GS-TBLB combined with ROSE group and CT-PNB group can be used as diagnostic techniques for peripheral lung cancer.Two biopsy methods have a high positive rate for the diagnosis of peripheral lung cancer.As a minimally invasive technology,EBUS-GS-TBLB combined with ROSE technology is not only very safe,but also does not have many complications.
