Study On The Attention Bias Of Post-stroke Depression Patients

With the improvement of medical and health level,the cure rate of stroke patients has been greatly improved.But for this kind of patient's psychological condition,the medical staff,the patient's family member even the patient itself pays little attention.Post-stroke depression(PSD)is a common mood disorder after stroke,which not only delays the recovery of stroke patients,but also may lead to patients' self-injury or even suicide,seriously affecting family harmony and social stability.Therefore,the assessment of depression in stroke patients and the analysis of its influencing factors are conducive to the early diagnosis and intervention of post-stroke depression.In addition,the clinical manifestations and physiological basis of post-stroke depression are very similar to endogenous depression,and depressed patients have obvious attention bias effect on negative stimuli,and this negative attention bias is an important reason for the generation,maintenance and development of depression.Therefore,exploring the attention bias of patients with post-stroke depression on emotional information is also conducive to the follow-up of more targeted psychological intervention in clinical practice,so as to promote the remission of depressive symptoms in patients.Based on this,we conducted the following two studies:Research one,through general situation questionnaire,9 items health questionnaire,Hamilton depression rating scale,Perceived social support scale,Rosenberg self-esteem scale,Barthel index and Mini-Mental State Examination,investigating the incidence and influencing factors of depression in 61 patients with acute stroke admitted to the department of neurology of wuhan zhongnan hospital and the first hospital since December 26,2018 to January 16,2019.The results showed that ?the incidence of PSD in stroke patients was 57.38%,which was at a high level.?The occurrence of post-stroke depression is the result of the combined action of biological,psychological,social and many other factors,which is related to daily life ability(ADL),cognitive function(MMSE),self-esteem(SES)and the presence or absence of diabetes,coronary heart disease,and diabetes is an independent risk factor for predicting PSD,ADL and SES are protective factors of PSD.Research two,the images of happy,sad and neutral faces that met the criteria were selected from the Chinese emotional face image database as stimulus materials,and the attentional bias characteristics on emotional faces in patients with post-stroke depression,patients with post-stroke non-depression and normal subjects were investigated using the cue-target task paradigm.The results showed that ?there was a significant difference in response time of the PSD group,non-PSD group and normal group in the cue-target task,and the response time of the PSD group was significantly longer than that of the non-PSD group and the normal group,indicating that the PSD group had attention deficit.?The PSD patients show an obvious attention bias toward the sad face,which was mainly due to the difficulty of disengaging attention from the sad face rather than being alert.In addition,the attentional avoidance tendency of happy faces may also aggravate their negative attentional bias effect.?IOR effects on sad faces were significantly less in the PSD and non-PSD groups than in the normal group,which to some extent explains the difficulty of disengaging attention from the sad faces in the two groups,indicating that the inhibition mechanism may play a more important role in the negative attention bias of PSD patients compared with the facilitation mechanism of attention.
