Governance Path Of Doctor-Patient Trust Relationship

Facing the reality that doctor-patient trust is frustrated and doctor-patient relationship is tense,the effective and feasible measurement scale is just like a barometer.It can provide a scientific tool to grasp the doctor-patient trust situation and measure the degree of doctor-patient trust.It can intervene precisely from the measurement of doctor-patient trust,find the shortcomings through the measurement of doctor-patient trust,and put forward the management path pertinently,so as to promote doctor-patient trust.Undoubtedly,the doctor-patient trust scale can create beneficial effects for building a good and stable doctor-patient trust relationship.To make Sachiko's“the four quadrant model”can be used,the research applied it as theoretical framework to form a scale about patient's trust in doctor by literature reviewing and Delphi expert consultation method.The model including eight dimensions,33 level at the end of the indicators and 65 measuring items.Afterwards,the scale was adjusted through field inspection,and the final index was cancelled.Finally,the scale included eight dimensions and 65 measurement items,which directly reflected the one-dimensional nature of the eight dimensions.The reliability and validity of the final scale were tested by Mplus8.0statistical analysis method in three grade a general hospitals.The total Cronbach's value of the scale was 0.951.The split-half reliability R was 0.818.The factor loading coefficients of65 items were all between 0.591-0.920.The value range of construct reliability of each dimension is between 0.8821 and 0.9736,with good content validity.Meanwhile,the structural validity of?~2/df,RMSEA,CFI and TLI was 2.236,0.04,0.945 and 0.922,so they are in the acceptable range.The scale has good reliability and validity.Finally,the empirical results show that:Surveyed patients evaluation of doctor-patient trust the mean score of341.997,up to 75.16%of the total score is full score,but from the average score of doctor-patient trust and in terms of average score of each dimension of doctor-patient trust are divided into 5.32,each dimension of average range between 3.93 to 6.63.The highest divide dimension is communication(6.63),the second is the competence(5.77)and confidence(5.50),system trust(5.54)and fidelity(5.40),but honesty(5.32),confidentiality(4.53)and fairness(3.93)have lower scores and lower than the average of doctor-patient trust.The overall evaluation of doctor-patient trust is good,and the trust content of different dimensions shows different levels of trust.This can provide ideas for searching the specific governance path of doctor-patient trust relationship,mobilizing systematically from macro to meso to micro,and taking the honesty degree of doctors to patients as the starting point to improve doctor-patient trust relationship,so as to promote the fairness of doctors to patients.In order to promote the breakthrough of medical trust relationship and improve the confidentiality of doctors to patients as an indirect way to improve doctor-patient trust relationship,in order to enhance doctor-patient trust and achieve harmonious doctor-patient relationship.
