Clinical Observation Of Triple Therapy For Recurrent Low Back Pain After Intervertebral Foramen Microscopy For Lumbar Disc Herniation

Objective:The purpose of this study is to study the clinical effect of triple therapy on recurrent low back pain in patients with lumbar disc herniation after percutaneous transforaminal endoscopic surgery,and to explore the possible mechanism of Wechsler triple therapy for recurrent low back pain after operation,so as to provide a new possibility for traditional Chinese medicine conservative treatment of recurrent low back pain after transforaminal endoscopic surgery.Methods:A retrospective analysis was made on 20 patients with lumbar disc herniation diagnosed in the first and second regions of the spine of** Hospital Affiliated from 2016 to 2019,who met the surgical indications and received consent from the patients and were followed up for recurrent low back pain.All the 20 patients need to be hospitalized again.On the basis of the same general treatment,Wechsler's triple therapy was started on the first day of admission: once a day,five consecutive days as a cycle,and two days' rest after one cycle.The treatment lasted for two cycles.Visual Analogue Scale(VAS),OswestryDability Index(ODI)and modified MacNab standard were evaluated on the first day,the seventh day,the fourteenth day and three months after treatment.The data were analyzed statistically,and the observation indexes before and after treatment were compared,and the treatment was analyzed Clinical effect.Results:1.VAS score comparison: Data comparison between the two groups before and after treatment: After the intervention of triple therapy,the VAS score showed a downward trend after treatment compared with that before treatment except the first day.There was no significant difference in VAS score between the first day after treatment and that before treatment(P=0.062,P>0.05),and there was significant difference in VAS score between the seventh day after treatment and that before treatment(P=0.000,P<0.05),and the 14 th day after treatment compared with that before treatment(P=0.000,P<0.05).There was significant difference in VAS score between day and before treatment(P = 0.000,P <0.05).There was significant difference in VAS score between 3 months after follow-up and before treatment(P = 0.000,P < 0.05).It shows that Wechsler's triple therapy has obvious effect on improving pain treatment.2.ODI score comparison: Before and after treatment,the ODI scores of the two groups showed a downward trend after treatment compared with those before treatment except the first day.There was no significant difference between the first day after treatment and that before treatment(P = 0.083,P > 0.05),and the seventh day after treatment had significant difference compared with that before treatment(P = 0.000,P < 0.05),and the fourteenth day after treatment was compared with that before treatment(P = 0.000,P < 0.05).There were significant differences in ODI scores before treatment(P = 0.000,P < 0.05),and there were significant differences in ODI scores after 3 months of follow-up and before treatment(P = 0.000,P < 0.05).It shows that Wechsler's triple therapy has obvious effect on recovery of lumbar vertebral dysfunction.3.Evaluation of improved MacNab standard:Twenty patients selected in this project were evaluated according to the improved MacNab standard after treatment.Through the comprehensive analysis of the course of disease,physical examination,symptoms changes before and after complaint and physical examination,18 cases were excellent,1 case was good,1 case was fair,1 case was poor,and the overall excellent and good rate was 95%(19/20).After treatment,1 case had no significant improvement in lumbar pain symptoms,and the patient voluntarily asked to give up manipulation.Continuing treatment,patients were given traditional fenestration operation after discussion and consent of the department.Symptoms were completely relieved and discharged after safe recovery.It shows that triple therapy has a good therapeutic effect on recurrent low back pain after intervertebral foramen surgery for lumbar disc herniation.Conclusion: The symptoms and signs of patients with recurrent low back pain after intervertebral foraminoscopy for lumbar disc herniation have been significantly improved by triple therapy.However,the present situation of recurrent low back pain after intervertebral foraminoscopy for lumbar disc herniation still exists,which provides the corresponding clinical support for traditional Chinese medicine conservative therapy and is worthy of clinical promotion.
