Objective: To observe the clinical efficacy and safety of Qiling Jianpi Granule in treating nephrotic syndrome of deficiency of spleen and kidney.Method: According to the inclusion and exclusion criteria of this study,patients with spleen and kidney deficiency nephrotic syndrome were randomly divided into two groups,30 cases in each group.The control group was treated with general symptomatic treatment and hormone standard treatment.The treatment group was treated with Qiling Jianpi Granule on the basis of the control group.The treatment lasted for 4 weeks,and the two groups were treated for 2 consecutive courses.The total effective rate,TCM syndrome score and laboratory examination indexes such as urinary routine,24-hour urinary protein quantification,plasma albumin,cholesterol,triglyceride and serum creatinine were observed after treatment.The adverse reactions and recurrences in the treatment were observed and recorded.Result: 1.There was no significant difference in gender,age and course of disease between the two groups(P>0.05).2.On the curative effect of TCM syndromes:The total effective rate was 93.33% in the treatment group and 70% in the control group.The treatment group was significantly better than the control group(P<0.05),and the recurrence rate in the treatment group was significantly lower than that in the control group(P<0.05).3.TCM main symptoms,syndrome curative effect analysis: the two groups TCM main symptom integral has the improvement,the treatment group main symptom performance is facial floating body swelling,the spirit fatigue,the epigastric fullness,the waist and knee acid soft integral drop degree is obviously better than the control group(P<0.05).There was no significant difference in face color between the two groups(P>0.05).The total effective rate was 90% in the treatment group and 66.67% in the control group.The curative effect in the treatment group was better than that in the control group(P<0.05).4.Analysis of urinary protein: There was significant difference between the two groups after treatment(P<0.01),indicating that the two groups could reduce the urine protein,and the treatment group was better than the control group in improving the urine protein(P<0.05).5.Analysis of 24-hour urinary protein quantity and plasma albumin: After treatment,the urinary protein content in the two groups decreased(P<0.01)and the plasma albumin content increased(P<0.01).The improvement of the two indexes in the treatment group was better than that in the control group(P<0.01).6.Analysis of the results of total cholesterol and triglyceride: the total cholesterol and triglyceride in the two groups were decreased(P<0.01),and the improvement of the two indexes in the treatment group was better than that in the control group(P<0.05or<0.01).7.Analysis of serum creatinine: the serum creatinine levels of the two groups were within the normal range,and there was no significant difference between the two groups(P>0.05).8.Analysis of safety result: ECG,blood routine,fecal routine,liver function and renal function were not abnormal in the two groups.No adverse reactions occurred.Conclusion: 1.Qiling Jianpi Granule combined with standard western medicine can effectively improve the clinical symptoms of patients with nephrotic syndrome;2.Qiling Jianpi Granule combined with standard western medicine can reduce urinary protein,total cholesterol and triglyceride content and increase plasma albumin level in patients with nephrotic syndrome;3.Qiling Jianpi Granule combined with standard treatment of western medicine can reduce the recurrence rate and is safe.There are no obvious adverse reactions in the course of the study.
