Objective:This thesis is to evaluate the clinical efficacy,safety and feasibility of the two surgical methods which axillary thoracotomy and video-assisted thoracoscope for non-small cell lung cancer in Qinghai area by analyzing the perioperative clinical indicators during operation,which will help the patients to choose a suitable surgical method in the course of treatment.Methods:A retrospective analysis was made of 60 patients who met the inclusion and exclusion criteria hospitalized in the Department of Thoracic and Cardiovascular Surgery of Affiliated Hospital of Qinghai University from September 2016 to September 2018.They were divided into two groups,30 in the open group(axillary mini-incision patients)and 30 in the thoracoscopy group,of whom the perioperative indicators were analyzed,such as intraoperative blood loss,1-3 days closed thoracic drainage volume,dosage of painkillers,antibiotic use,incision infection,postoperative fever,the total in-patient expenses and the day staying hospital after operation.SPSS 24.0 software was used to analyze the data.The results showed that P value<0.05,which indicated that there were differences,and there was statistical significance.Results:1.There was no statistical significance between the two groups in gender,age,Pathological type and the location of tumor growth(P>0.05).2.The blood loss volume during operation in open group was 320(200?510)ml,while in thoracoscopy group was 110(75?142.5)ml.and the blood loss volume in open group was more than that in thoracoscopy group.There was statistical difference(P<0.05).3.Closed thoracic drainage volume in open group was 600(400?700)ml in 1-3 days,while in thoracoscopy group was 210(171.25?242.5)ml,and the drainage in open group was more than that in thoracoscopy group.There was statistical difference(P<0.05).4.The dosage of analgesics in the open group was 255(202.5?393.75)ml,120(60?210)ml in the thoracoscopy group.Statistical analysis showed that the dosage of analgesics in the open group was significantly higher than that in the thoracoscopy group(P<0.05).5.Antibiotic use time in open group:8(7?10)days,In thoracoscopy group:6(5?7)days.Statistical analysis P<0.05,with statistical difference.6.Incision infection was found in 8 cases(26.67%)in the open group and 1 case(3.33%)in the thoracoscopy group.There were more cases in the open group than in the thoracoscopy group,Statistical analysis P<0.05,with statistical difference.7.Fever occurred in 4 cases(13.33%)in the open group and 1(3.33%)in the thoracoscopy group.The fever rate in the open group was higher than that in the thoracoscopy group,Statistical analysis P>0.05,without statistical difference.8.The post operative hospital time of open group was 11.5(9?14.25)days,while thoracoscopy group was 9(7.75?10)days.The time of open group was longer than that of thoracoscopy group,Statistical analysis P<0.05,with statistical difference.9.The total in-patient expenses in the open group were(59289.98 ±11761.13)?,while in the thoracoscopy group(64.39±8635.46)?.The expenses in the open group were less than that in the thoracoscopy group.Statistical analysis P<0.05,with statistical difference.Conclusion:Video-assisted thoracoscopic surgery can significantly reduce intraoperative blood loss,postoperative drainage volume,dosage of analgesics,antibiotics use,incision infection and post operative hospital time in the treatment of non-small cell lung cancer.However,axillary mini-incision can save hospitalization costs.There is no significant difference in fever after operation between the two treatments.
