Axillary osmidrosis(AO)is a common disease in the clinic,and the proportion of patients who seek treatment in plastic surgery should not be ignored.There are many clinical methods of treatment,of which the most exacting effect is surgical treatment.However,the traditional surgical excision has great damage,and the removal of skin can easily lead to postoperative complications such as limited mobility of upper limbs,causing great inconvenience to patients.Therefore,in recent years,minimally invasive surgery has been used to remove the apocrine gland to reduce postoperative injuries,so as to preserve the local skin and ensure the upper limbs move freely.Minimally invasive surgery includes apocrine glands pruning,suction curettage and liposuction,and apocrine glands pruning under direct vision is common in clinical.The postoperative recurrence rate of it is the lowest in the three methods,but the postoperative healing is slow and the complication rate is higher.Therefore,this study is mainly aimed at two different incisions-commonly used in apocrine glands pruning-axillary folds parallel double incision and transverse incision,retrospective analysis of the difference between the two surgical procedures for early healing and late complications.Objective:This study used the axillary folds parallel double incision and transverse incision in treatment of axillary osmidrosis.We have a contrastive study from early postoperative complications(subcutaneous ecchymosis,hematoma,infection,wound dehiscence,exfoliation and necrosis)and late complications(arm numbness,scar contracture,odor residue and recurrence),in order to provide guidance in clinical treatment and improve the satisfaction of patients.Methods:A retrospective analysis was performed on the patients with bilateral armpits in the plastic surgery department of the China-Japan Friendship Hospital Affiliated Jilin University from September 2015 to June 2017,with 96 cases in accordance with the inclusion criteria.Accept the axillary folds parallel double incision is as group A,and accept the transverse incision below the edge of armpit hair for B group.The operation was performed with small incision and sweat gland pruning combined with electric ion burning.After 6 months of follow-up,19 cases were lost and 77 cases were followed up.The basic data of the patients were collected,and the early healing conditions and late postoperative were recorded and compared.Results:The samples were allocated into two groups: the axillary folds parallel double incision group(A group)and transverse incision group(B group).Two groups were analyzed statistically.No significant correlation was found between gender,age and the degree of axillary osmidrosis.And the operation process is the same.After the operation,the two groups of A and B were followed up early and late,and the healing conditions and curative effect were observed.Early observations showed subcutaneous ecchymosis,hematoma,incision split,epidermis shedding in groups A and B appeared and faded at different times.Group A subcutaneous ecchymosis faded mainly concentrated in six to eight days postoperatively,and group B mainly concentrated in 4 to 6 days;In group A,there were 2 cases of hematoma,3 cases of incision cleft,3 cases of epidermal detachment,0 cases of infection and 0 cases of necrosis.In group B,there were 1 case of hematoma,1 case of incision,1 case of epidermis,0 cases of infection and 0 cases of necrosis.Healing time of group A was concentrated in 12-14 days,and group B was concentrated in 10 days.Late observations showed 2 cases of armpit numbness,3 cases of scar contracture were observed in group A,and the cure rate was 81%.In group B,1 case of numbness of armpit and 1 case of scar contracture were observed and the cure rate was 85.7%.The recurrence rate of Group A was 7.14% and Group B was 5.71%.In group A,the incidence of complications in early and late stage was 23.8%,and that in group B was 11.4%.Conclusion:(1)For postoperative healing,the axillary folds parallel incision relative to the transverse incision is more likely to appear postoperative complications,and has longer duration and longer healing time.(2)For efficacy,the cure rate and the recurrence rate of axillary folds parallel double incision have no different from that of transverse incision.
