Objective:Accurate assessment of the efficacy of neoadjuvant chemotherapy(NAC)after NAC in breast cancer determines the choice of surgical approach,determines the extent of resection of the breast tissue and the clearance of axillary lymph nodes.Currently there is no consensus on the best diagnostic method;A systematic search of the database,based on the original study of the search,we conducted a systematic review and meta-analysis of published studies to assess the accuracy of magnetic resonance imaging(MRI)and positron emission computed tomography(PET/CT)in the diagnosis of pathological response after breast cancer NAC.Methods : The Cochrane Library,MEDLINE,Embase,Web of Science,China Biomedical Literature Database(CBM),Chinese Science and Technology Journal Full-text Database(VIP),Wanfang Database were searched by searching all the literatures that included information on the efficacy of breast cancer NAC.Screen and incorporate the original studies that meet the requirements,analyze the original studies,extract the data,summarize the sensitivity and specificity of each study,plot the ROC curve,calculate the area under the curve,and compare the diagnostic value of MRI and PET/ CT.Results:Thirteen studies involving 575 patients who underwent MRI and 618 who underwent PET/CT were included in our analysis.The pooled sensitivity and specificity of MRI were 0.88(95% CI: 0.78-0.94)and 0.69(95% CI: 0.51-0.83),respectively.The corresponding values for PET/CT were 0.77(95% CI: 0.58-0.90)and 0.78(95% CI: 0.63-0.88),respectively.The area under the ROC curve for MRI and PET/CT were 0.88 and 0.84,respectively.Conclusion:MRI is more sensitive than PET/CT in predicting the pathological response after breast cancer in patients with breast cancer,but the specificity is lower than PET/CT.According to the results of ROC curve analysis,MRI is more accurate than PET/CT diagnosis.According to the comprehensive consideration of diagnostic accuracy and cost,we recommend patients to use MRI.
