| Objective To explore the correlation between lung-kidney-yin deficiency syndrome and blood gas analysis in stable stage of COPD by prospective investigation.Based on the understanding of the deficiency of chronic obstructive lung syndrome,the correlation between the degree of yin deficiency and blood gas analysis and the degree of lung function impairment in the syndrome of lung-kidney-yin deficiency was further discussed,which provided the basis for the clinical treatment of chronic obstructive lung of lung-kidney-yin deficiency type.Methods To collect the general internal medicine ward of XX Institute of traditional Chinese Medicine,respiratory out-patient department,and the patients who were diagnosed as chronic obstructive pulmonary disease(COPD)at the same time in the stable period of COPD,according to the syndrome differentiation and classification according to the four diagnostic parameters of TCM at the first time.Fifty-three patients with lung-kidney yin deficiency syndrome were selected.The general information data(including height,age,weight,sex)and four diagnostic data were collected to score and record the symptoms of lung-kidney yin deficiency syndrome.Blood gas analysis and pulmonary function test were performed on the patients without any treatment(including oxygen inhalation).The results of blood gas analysis and pulmonary function classification were recorded.Statistical data were divided into three groups according to the score results of patients’symptoms(grouping criteria:(maximum-minimum)/3),which were divided into three groups:Yin deficiency mild group,Yin deficiency moderate degree group,Yin deficiency severe group,SPSS24.0 statistical software was used to analyze the above-mentioned data.Results1.There was no significant difference in sex composition,age,weight and height among the three groups(P>0.05),and there was no significant difference between the two groups(P>0.05).2.In chronic obstructive pulmonary syndrome of lung-kidney-yin deficiency,the degree of lung function impairment was different among the groups with different degrees of yin deficiency,and the difference between groups was significant(P<0.05).3.There was no significant difference in the frequency of respiratory failure among the groups with different degrees of yin deficiency in chronic obstructive lung,lung and kidney yin deficiency syndrome(p>0.05).4.There were significant differences in PaCO2 and HCO-,CO CP values between groups with different degrees of Yin deficiency.Comparison between PaCO2 group:there was no significant difference between the mild Yin deficiency group and the Yin deficiency moderate degree group(P>0.05),but there was no statistical difference between the mild Yin deficiency group and the severe Yin deficiency group,and the comparison between the Yin deficiency mild group and the severe Yin deficiency group,and between the Yin deficiency moderate degree group and the Yin deficiency moderate degree group(P<0.05).HCO3-group comparison:Yin deficiency mild group and Yin deficiency moderate group,P>0.05,there was no statistical significance,Yin deficiency mild group and severe Yin deficiency group,P>0.05,there was no significant difference between Yin deficiency mild group and Yin deficiency severe group,P>0.05,there was no significant difference between Yin deficiency mild group and Yin deficiency moderate degree group(P>0.05).There was a significant difference between the severe Yin deficiency group and the Yin deficiency moderate degree group(P<0.05).The comparison between the CO2cp group:the Yin deficiency mild group and the Yin deficiency moderate group,P>0.05,there was no significant difference between the mild Yin deficiency group and the severe Yin deficiency group,and there was no significant difference between the mild Yin deficiency group and the severe Yin deficiency group,and there was no significant difference between the two groups.There was a significant difference between the severe Yin deficiency group and the Yin deficiency moderate degree group(P<0.05),and there was a significant difference between the severe Yin deficiency group and the Yin deficiency moderate degree group(P<0.05).Conclusion1.In the course of aggravation of Yin deficiency degree,carbon dioxide retention gradually aggravated,metabolic alkalinity and respiratory acidity would aggravate when the light degree of yin deficiency was changed from lung and kidney yin deficiency to a severe degree.2.There were significant differences in PaCO2,HCO3-,CO2CP among the groups with different degrees of yin deficiency in chronic obstructive lung,lung and kidney yin deficiency.In mild yin deficiency,PaCO2,HCO3-,CO2CP and other indexes changed little with the aggravation of yin deficiency,but when the severity of yin deficiency was more severe,PaCO2,HCO3-,was not obvious.CO2CP increased with the severity of Yin deficiency. |
