| ObsjectiveThrough clinical prospective randomized controlled study,the clinical efficacy and the level of serum TNF-α and IL-8 based on the method of autologous whole blood acupoint injections for treating in stable COPD patients with Qi deficiency of the lung and kidney syndrome were observed.MethodFrom October 2018 to February 2019,a randomized controlled trial was conducted in the inpatient or outpatient department of Respiratory Department,the first affiliated Hospital of Guangzhou University of traditional Chinese Medicine.60 patients with stable COPD were diagnosed by pulmonary function tests,metting the criteria of inclusion,exclusion and exclusion.The control group was given basic treatment of western medicine,and the treatment group was given self-blood point injection therapy on the basis of basic treatment of western medicine,choosing the Dingchuan acupoint、Feishu acupoint、Fenglong acupoint、Shenshu acupoint、Guanyuanshu acupoint,a group per time,once injection every other day,a course of 4 times.The changes of the level of TNF-a and IL-8 in peripheral blood and scale scores in the two groups were observed after 50 days.Results1.Effect on clinical efficacy:The total efficiency of the treatment group was 86.7%,while the total effective rate of the control group was 73.3%,the total efficiency of treatment group was higher than control group,but the difference was not significant(P>0.05).2.CAT score:After treatment,the difference of CAT score between the control group and the treatment group was statistically significant(P<0.05),which is all decreased compared with those before;Compared the CAT score difference between the two groups before and after treatment,the CAT score difference in the treatment group were significantly more than control group(P<0.05).3.mMRC score:After treatment,the mMRC score in the treatment group decreased compared with before,the difference was statistically significant(P<0.05).But there was no obvious differences in the control group decreased compared with before(P>0.05).4.6 minutes walk distance(6MWD):After treatment,the 6MWD in the two groups was higher than before the treatment(P<0.05),and the treatment group was more obvious higher than the control group(P<0.05).5.Lung function assessment:After treatment,the FVC and FEV1%pre of the treatment group and the FVC of the control group were higher than before,the difference was statistically significant(P<0.05),but the FEV1%pre of the control group the FEV1/FVC of both group had no obvious change,there is no statistical significance(P>0.05).The difference between the two groups before and after treatment,the FVC of treatment group was significantly higher than that in the control group,there was statistically significant(P<0.05),while there was no significant difference between FEV1%and FEV1/FVC before and after treatment in two groups(P>0.05).6.Comparison of TCM Syndrome Scale of each symptom score:After treatment,the symptom scores of the treatment group were lower than that before treatment is cough,phlegm,wheezing,shortness of breath,fatigue,chills and clear abundant urine,there was statistical significance(P<0.05),but there was no obvious differences in weakness of the lumber region and knee joint as well as cold extremities(P>0.05).After treatment,the symptom scores of the lumber region and knee joint,chills,cold extremities and clear abundant urine in control group were no obvious differences((P>0.05),but there were significantly decreased in cough,phlegm,wheezing,shortness of breath and fatigue(P>0.05);The symptom score of cough,fatigue and clear abundant urine in treatment group decreased more significantly than the control group,the difference was statistically significant(P<0.05)7.Serum TNF-a and IL-8:After treatment,the level of TNF-a and IL-8 in treatment group and control group were significantly decreased compared with before,the difference was statistically significant(P<0.05),and there was significant difference between the two groups before and after the treatment of TNF-a and IL-8(P<0.05).8.Adverse reactions:the two groups of patients in the course of treatment were no adverse reactions.ConclusionThe study show that the combination of autologous whole blood acupoint injections and conventional western medicine treatment in treating stable COPD patients with Lung and Kidney deficiency,can significantly improve their clinical symptoms as well as the survival quality and force their vital capacity,enhance the total effective rate of clinical treatment,whose therapeutic mechanism may be related to debase the level of TNF-α and IL-8. |
