Objective: To observe the clinical effect of traditional Chinese medicine Hot-Pack assisted percutaneous transforaminal endoscopy discectomy in the treatment of lumbar disc herniation.Methods: 60 patients with lumbar disc herniation admitted to our hospital from January 2018 to July 2018 were randomly divided into control group and treatment group according to the serial number of operation time according to the random number table method,and 30 cases in each group.The treatment group was treated by traditional Chinese medicine Hot-Pack assisted by percutaneous transforaminal endoscopy discectomy,while the control group was treated by hot water bag assisted by percutaneous transforaminal endoscopy discectomy.The patients in both groups were instructed to rest in bed after operation to guide functional exercise.The total follow-up period was 3 months.Visual analogue pain scale(VAS)was used to evaluate the lumbar pain,lumbar JOA score was used to evaluate the improvement of lumbar function and Oswestry dysfunction index score was used to evaluate the improvement of quality of life of patients before and after treatment on the 1st,1 month,2 months and 3 months respectively.The scores of the above scale were collected and recorded through outpatient,inpatient or follow-up.Modified Macnab criteria were used to evaluate the clinical efficacy at 3 months after operation.Results: A total of 60 patients were enrolled.The follow-up period was 3 months.All patients were able to cooperate with the treatment and successfully complete the study.There were 30 cases in the control group,with an average age of(41.37±12.67)years and an average course of disease of(23.58±28.08)months.There were 2 cases,19 cases and 9 cases in L3-4,L4-5 and L5-S1.There were 30 cases in the treatment group,with an average age of(42.4±10.45)years and an average course of disease of(28.72±35.83)months.There were 2 cases,23 cases and 5 cases in L3-4,L4-5 and L5-S1 respectively.The VAS scores of low back pain in the two groups at each follow-up time point were significantly lower than those before operation(P<0.01).The VAS scores in the treatment group were significantly lower than those in the control group at 1 month,2 months and 3 months after operation(P<0.01).The JOA scores at each follow-up time point in the two groups were significantly higher than those before operation(P<0.05).The JOA scores in the treatment group were significantly higher than those in the control group at 1 month,2 months and 3 months after operation(P<0.01).The ODI index of all follow-up time points after operation was significantly lower than that before operation.The ODI index of treatment group was significantly lower than that of control group at1 month,2 months and 3 months after operation(P<0.05).The total excellent and good rates of the treatment group and the control group were 93.3% and 90.0% respectively at 3 months after operation,and there was no significant difference between the two groups(P > 0.05).One patient in the treatment group had excessive local bleeding in the surgical incision after operation,and was treated with local pressure dressing after changing dressings without obstruction,so he continued to be included in the treatment group of this study.In the treatment group,there were no local skin burns,ulceration and allergic itching.Conclusion: Traditional Chinese medicine Hot-Pack plays an active role in improving early intervention after surgery for lumbar protrusion by foramen endoscopy,which is helpful to relieve the symptoms of residual lumbar pain after operation and to accelerate the recovery of body function after operation.
