Controlled Clinical Studie Of Micro Endoscopic Discectomy Combined With Fiber Ring Suture And Percutaneous Endoscopic Lumbar Discectomy In Treatment Of Single Lumbar Disc Herniation

Objective To investigate the clinical effect of micro endoscopic discectomy with annulus repair and percutaneous endoscopic lumbar discectomy in the treatment of single segment prolapse of lumbar intervertebral disc.Method A total of 80 patients with single segment prolapse of lumbar intervertebral disc in our hospital from January 2015 to January 2016 were equally divided into two groups,micro endoscopic discectomy with annulus repair(annulus repair group)and percutaneous endoscopic lumbar discectomy group(PELD group)which underwent micro endoscopic discectomy with annulus repair and percutaneous endoscopic lumbar discectomy separately.The process of the treatment in hospital,Visual analogue scale,Oswestry disability index,Modified Macnab criteria were used to evaluated the clinical effect.Result Two groups of patients were successfully completed surgery and follow-up,postoperative follow-up of 12 to 22 months,an average of 16 months.The average incision of annulus repair group was(18.27±1.20)mm,surgical blood loss was(54.75±9.47)ml,operative time was(73.25±7.89)min,postoperative hospital stay was(7.38±0.95)d.The average incision of PELD group was(7.30±0.79)mm,surgical blood loss was(6.85±1.23)ml,operative time was(59.13±8.61)min,postoperative hospital stay was(6.10±0.74)d.The VAS and ODI of the two groups were improved obviously after surgery.In 1 year postoperative follow-up,there was no statistically significant difference of VAS and ODI and Modified Macnab criteria between two groups of patients.33 patients of annulus repair group were successfully sutured,the sutured rate was 82.5%,1 recurrence case was improved after symptomatic treatment.40 patients of PELD group were successful surgery,5 recurrence cases,4 cases under PELD again and 1 case was improved after symptomatic treatment.Conclusion Micro endoscopic discectomy with annulus repair is a bit poor in incision,surgical blood loss,operative time postoperative hospital stay.Micro endoscopic discectomy with annulus repair has certain failure rate in the treatment of single segment prolapse of lumbar intervertebral disc and it is necessary to further research and improvement.
