ObjectiveTo study the occurrence and related factors of anxiety disorder in patients with T2DM,in order to find out and intervene the anxiety disorder of T2DM patients as early as possible,and to provide a reference for clinical treatment to improve the mental health of T2DM patients.Methods(1)Research object:We adopted a cross-sectional study method and selected 295 patients with T2DM in the Endocrinology Department of Shaanxi Provincial people's Hospital from March 2018 to October 2018.(2)Data collection:Collect clinical baseline data of all included populations,Including gender,age,height,weight,body mass index(BMI),waist circumference,the course of diabetes,the number of diabetes complications,the times of daily insulin injection,glycated hemoglobin(HbA1C),blood routine,blood lipid series,renal function,liver function and other indicators.(3)Anxiety assessment:The anxiety was assessed by using the Hamilton Anxiety Scale(HAMA,including 14 items,with a score of less than 7 indicating no anxiety disorder,a score of 7 to 14 indicating possible anxiety disorder,a score of 15 to 21 indicating positive anxiety disorder,a score of 22 to 29 indicating significant anxiety disorder,and a score of more than 29 indicating severe anxiety disorder.The HAMA score of more than 14 was defined as an anxiety disorder).People with an HAMA score of more than 14 are defined as having an anxiety disorder.(4)Statistical methods:All data were analyzed statistically using SPSS19.0 statistical software.The measurement data in line with the normal distribution were expressed as mean±standard deviation(x±s),and the measurement data not conforming to the normal distribution were expressed as logarithmic transformation.The counting data were expressed as percentage.Patients with T2DM were divided into anxiety group and non-anxiety group according to anxiety score.The t test was used for the measurement between the two groups,and?~2 test was used for the counting data.Pearson correlation analysis was used to explore the correlation between anxiety score and various factors in patients with T2DM.Multiple linear regression analysis was used to explore the related factors of anxiety disorder in T2DM patients.Results(1)Among the patients with T2DM,233(78.98%)had no anxiety disorder and 62(21.02%)had anxiety disorder.(2)The ages of the anxious group and the non-anxious group were 62.94±7.82(years)VS 61.57±9.62(years).The number of women(percentage)were 31(50%)VS 78(33.5%);The course of diabetes was 11.29±2.89(years)VS 8.65±4.64(years).The number of diabetic complications was 3.39±0.86 VS 1.90±0.90.The times of daily insulin injection were 1.19±1.39 VS 0.78±1.27.The number of people with hypertension(percentage)were41(66.1%)VS 123(52.8%).NEUT percentage(NEUT)was 0.64±0.08 VS 0.61±0.07.Blood urea nitrogen(BUN)was 6.65±2.72(mmol/L)VS 5.91±1.56(mmol/L).Serum creatinine(CR)was 80.87±41.72(umol/L)VS 67.21±17.49(umol/L).Cystatin C(CYS-C)was 1.12±0.44(mg/L)VS 1.00±0.26(mg/L).Albumin(ALB)was 38.84±4.23(g/L)VS39.96±3.58(g/L).HbA1C was 8.06±0.98(%)VS 7.75±1.27(%).P<0.05.(3)The Pearson correlation analysis between anxiety score and each index showed that anxiety score was positively correlated with age(r=0.250,P<0.05),and positively correlated with the course of diabetes(r=0.431,P<0.05),and positively correlated with the times of daily insulin injections(r=0.255,P<0.05),and positively correlated with the number of diabetes complications(r=0.483,P<0.05),and negatively correlated with red blood cell count(RBC)(r=0.153,P<0.05),and negatively correlated with,hemoglobin count(HB)(r=0.209,P<0.05),and positively correlated with total cholesterol(TC)(r=0.134,P<0.05),and positively correlated with CR(r=0.230,P<0.05),and positively correlated with CYS-C(r=0.165,P<0.05),and positively correlated with RBP(r=0.133,P<0.05),and positively correlated with HbA1C(r=0.162,P<0.05).(4)Multiple linear regression analysis between anxiety score and the included indicators showed that the multiple linear regression equation was:anxiety score=-7.837+0.062×age+0.447×the course of diabetes+0.942×the times of daily insulin injections+3.834×the number of diabetes complications+0.904×TC+0.055×RBP+0.636×HbA1C.Conclusion(1)The prevalence of anxiety disorder in T2DM patients was 21.02%;(2)Age,the course of diabetes,the times of daily insulin injections,the number of diabetic complications,TC,RBP and HbA1C are associated with anxiety disorder in patients with T2DM.
