ObjectivesBy comparing the application of conventional nursing mode and eCASH mode in ICU patients,the effects of the eCASH mode on ICU patients' muscle strength score,self-care ability score,incidence of acquired frailty in ICU,anxiety and depression score,incidence of delirium in ICU and clinical outcomes were analyzed,and the clinical effects of the eCASH mode in the prevention of post-intensive care syndrome were discussed.MethodsA total of 138 patients were selected as the research objects on the principle of convenience sampling,who admitted to the Affiliated Hospital of Jiangsu University in Zhenjiang City from June 2017 to June 2018 for comprehensive ICU treatment.The data of patients admitted to the ICU from June 2017 to December 2017 were retrospectively analyzed.The patients who met the inclusion criteria were selected as the control group(69 cases).The patients who were admitted to the ICU from January 2018 to June 2018 and met the inclusion criteria were selected as the experimental group(66 cases)according to the baseline data and matched with the control group.The control group was given routine ICU care.The experimental group was given eCASH mode nursing care.After enrollment,the general data and clinical data of patients were collected using the baseline data questionnaire,and the comparability of the data between the two groups was analyzed.At the time of ICU transfer and one week after ICU transfer,the Medical Research Council Muscle Rating Scale was used to compare the muscle strength and the incidence of intensive care unit-acquired weakness,the Barthel index was used to compare self-care ability score between the two groups.Hospital anxiety and depression scale was used to compare the scores of anxiety and depression between the two groups.The incidence of delirium in ICU was compared between the two groups using Confusion assessment method.The duration of mechanical ventilation,length of stay in the ICU,the rate of transfer into general ward,and mortality were assessed,and clinical outcomes were compared between the two groups.Results1.Comparison of baseline data: there were no statistically significant differences in general data and clinical data between the control group and the experimental group(P>0.05),and the baseline characteristics were similar and comparable.2.Comparison of muscle strength,self-care ability score and the incidence of intensive care unit-acquired weakness: the muscle strength score and the Barthel index of the experimental group were higher than those of the control group at the time of ICU transfer and one week after ICU transfer,and the difference was statistically significant(P<0.05).The incidence of intensive care unit-acquired weakness in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).3.Comparison of anxiety and depression scores: At the time of ICU transfer and one week after ICU transfer,anxiety and depression scores of patients in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05).4.Comparison of incidence of delirium in ICU: the incidence of delirium in ICU in the experimental group was lower than that in the control group,and the difference was not statistically significant(P>0.05).5.Comparison of clinical outcomes: the duration of mechanical ventilation and the length of stay in the ICU in the experimental group were shorter than that in the control group,with statistically significant differences(P<0.05).There was no significant difference in the rate of transfer into general ward,and mortality between the two groups(P>0.05).Conclusions1.The eCASH mode can improve the muscle strength of patients at the time of ICU transfer and one week after ICU transfer,improve the self-care ability,reduce the incidence of intensive care unit-acquired weakness,and enhance the body function;2.The eCASH mode can alleviate the anxiety and depression level of patients at the time of ICU transfer and one week after ICU transfer,and reduce the psychological impairment;3.The eCASH mode can shorten the duration of mechanical ventilation and the length of stay in the ICUand improves the clinical outcomes;4.Both the eCASH mode nursing and routine nursing methods are worthy of clinical promotion and application,and the application of eCASH mode can better improve the results related to the physical,psychology and cognition of patients.
