Objective:To study the level of preoperative social support in patients with colorectal cancer and its influencing factors,and to explore the relationship between preoperative social support and depression,anxiety,self-esteem,subjective well-being and quality of life in patients with colorectal cancer.To provide scientific basis for clinical medical staff to treat patients according to different levels of social support,and to provide theoretical basis for further improving the level of social support and quality of life of cancer patients.Methods:From September 2016 to December 2017,100 patients with preoperative colorectal cancer were enrolled in the Anorectal Surgery Department of the First Affiliated Hospital of China Medical University.General demographic information questionnaire,pathological typing data,Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),Social Support Scale(PSSS),Memorial University of Newfoundland Subjective Well-being Scale(MUNSH),Rosenberg Self-esteem Scale(SES),Quality of Life Scale for Cancer Patients(QLQ-C30)were used to evaluate preoperative general demography of colorectal cancer patients.Situation,pathological characteristics,depression and anxiety,self-esteem,subjective well-being and quality of life.According to the level of social support,100 patients were divided into low social support group,middle social support group and high social support group.The demographic and pathological characteristics of the three groups were compared by single factor analysis.Statistically significant factors were included in the ordered logistic regression model to analyze the independent factors affecting social support.Perason linear correlation analysis was used to analyze the correlation between social support and depression,anxiety,self-esteem,subjective well-being and quality of life,multivariate regression analysis was conducted with the factors related to social support as independent variables and social support from different sources respectively.Result:1.The preoperative social support score of colorectal cancer patients was(63.05±12.71).The three sources were: family support(22.09±3.98),friend support(19.42±5.25),others support(21.54±5.11).2.Univariate analysis found that the level of social support was related to educational level and gross pathological classification of tumors(P < 0.05).The higher the educational level,the higher the level of social support;ulcerative colorectal cancer patients had the highest level of social support.The level of social support was not related to age,sex,marriage,occupation,personal income,TNM stage and histological classification of tumors(P > 0.05).Ordered Logistic regression showed that education level was an independent factor affecting social support(P < 0.05).3.The results of correlation analysis showed that the preoperative total score of social support was negatively correlated with depression and anxiety(P < 0.05),but not with self-esteem(P > 0.05);the preoperative total score of subjective well-being of patients with colorectal cancer was positively correlated with the total score of social support and other people's support(P < 0.01),and positively correlated with family support and friend support(P < 0.05).Negative emotions were negatively correlated with total scores of social support,family support and other people's support(P < 0.01).Negative experiences were negatively correlated with total scores of social support and family support(P < 0.01),negatively correlated with other people's support(P < 0.05).Preoperative physical and social functions of patients with colorectal cancer were positively correlated with other people's support(P < 0.05),and role functions were positively correlated with other people's support(P < 0.05).Family support was positively correlated(P < 0.05),emotional function was positively correlated with friend support(P < 0.05),and cognitive function was positively correlated with family support and other support(P < 0.05).4.Regression analysis showed that depression and anxiety,subjective well-being,negative emotions and negative experiences all had an impact on the total score of social support of patients with colorectal cancer before operation(P < 0.01),but negative experiences had no significant impact on the total score of social support(P > 0.05);depression and anxiety,subjective well-being and negative emotions had an impact on the total score of social support of patients with colorectal cancer before operation(P < 0.01).Among them,negative emotion has the greatest negative effect.Anxiety,subjective well-being total score,negative emotion,negative experience and role and cognitive function all had influence on family support of patients with colorectal cancer before operation(P < 0.01).Anxiety,subjective well-being total score and negative experience had no significant influence on family support(P > 0.05).Negative emotion,role and cognitive function had significant influence on family support(P < 0.01).The negative effect of cognitive function is the greatest.The total score of subjective well-being and emotional function had an effect on friend support of patients with colorectal cancer before operation(P < 0.01),but the influence of emotional function on friend support was not significant(P > 0.05);the total score of subjective well-being had a positive effect on friend support(P < 0.05).The total score of subjective well-being,physical function,cognitive function and social function all had an effect on other people's support in patients with colorectal cancer before operation(P < 0.01),but only the total score of subjective well-being had a positive effect on other people's support(P < 0.05),and the others had no significant effect(P > 0.05).Conclusion:1.Educational level is an independent factor affecting preoperative social support for patients with colorectal cancer.The higher the educational level,the higher the level of social support.2.The higher the level of social support,the lower the degree of depression and anxiety,the higher the subjective well-being and quality of life of patients with colorectal cancer before operation.
