Objective:To understand the thyroid function of women in early pregnancy.To explore the relationship between thyroid dysfunction and missed abortion.To provide a reference for preventing the occurrence of clinical missed abortion.Analysis of clinical application value of thyroid function examination.Methods:The medical records of 112 patients with missed abortion and 249 normal pregnant women from September 2016 to September 2018 in our hospital were collected.The proportion of overt hypothyroidism(OH),subclinical hypothyroidism(SCH),hypothyroxinemia(HT),thyrotoxicosis,and thyroid autoantibody positive types in the two groups were analyzed.Comparison of t-tests of two independent samples to compare whether there are differences in thyroid detection indicators between case group and control group.Through?~2 test to analyze the differences in age,ethnicity,body mass index,maternal birth,cesarean section,abortion history,infection during pregnancy,and thyroid function.Logistic regression analysis was used as an independent variable to determine whether the risk of missed abortion in the area was included as a dependent variable,including factors such as age,maternal parity and thyroid function.Results:There were no significant differences in thyroid function between the missed abortion group and the normal pregnancy group about TT3(P>0.05),and there were statistical differences between TT4,FT3,FT4,TSH and TPOAb(P<0.05).Compared with the normal pregnancy group,the missed abortion group had no differences in ethnicity,body mass index,cesarean section history,abortion history,and infection during pregnancy,but there were differences in age,maternal parity,and thyroid dysfunction.Clinical hypothyroidism,subclinical hypothyroidism and thyroid antibody positive were significantly higher in the thyroid dysfunction than in the control group,and there was no difference between low T4 and thyrotoxicosis.Logistic regression analysis found that age-appropriate protection factors for missed abortion;clinical hypothyroidism,subclinical hypothyroidism,and thyroid antibody-positive in primary pregnancy and thyroid dysfunction were risk factors for missed abortion.Conclusions:Thyroid dysfunction,especially clinical hypothyroidism,subclinical hypothyroidism,and thyroid autoimmunity associated with missed abortion.Advanced age and primipara may increase the risk of missed abortion in patients with thyroid dysfunction.Age-appropriate was the protective factor for missed abortion.First pregnancy and clinical hypothyroidism,subclinical hypothyroidism and thyroid antibody-positive in thyroid dysfunction are risk factors for missed abortion.In clinical work,we should pay attention to the detection of thyroid function during early pregnancy,and formulate corresponding treatment plans according to abnormal conditions of thyroid function.
