Objective:In this study,we observed the clinical effect of transabdominal preperitoneal herniorrhaphy(TAPP)and ultrapro hernia system(UHS)herniorrhaphy in the treatment of elderly patients with emergency incarcerated hernia,and explored the safety of TAPP in the treatment of elderly patients with emergency incarcerated hernia.Methods:The subjects of this study are all from 95 elderly patients with incarcerated hernia who were treated by abdominal endoscopic surgery in the Affiliated Hospital of Qinghai University from September 2014 to September 2016.After preoperative evaluation,they were randomly divided into two groups,45 cases in TAPP group underwent transabdominal preperitoneal herniorrhaphy,and 50 cases in UHS group underwent ultrapro hernia system repair.Complications and long-term complications were compared between the two groups.The 95 patients selected in this study were successfully operated and discharged safely.Results:Both groups were followed up for 3 to 24 months(median 13 months).There was no significant difference in general information,basic diseases and complications between TAPP group and UHS group(P > 0.05).Because some elderly patients with emergency incarcerated hernia in the two groups had bilateral inguinal hernia(one side with incarceration)at the time of endoscopy exploration or admission,bilateral inguinal hernia was removed,and only unilateral inguinal hernia was compared for surgical treatment;The operation time of the two groups(85.26±7.92VS66.03±6.01)min,intraoperative blood loss(12.14±4.70VS19.53±5.43)ml,hospitalization time(4.50±1.47VS5.73±2.04)d,hospitalization expenses(13491.59±3119.77VS9578.83±423.03)yuan,the daily activity time was restored(1.85±0.83VS2.62±0.99)d,and the difference between the two groups was statistically significant(P<0.05).The VAS scores of the two groups were(2.07±0.084VS2.42±0.078)points,there were significant differences between the two groups(P < 0.05).Conclusion:TAPP is a viable and safe surgical procedure for the treatment of elderly patients with emergency incarcerated hernia.This procedure has the advantages of small trauma,less intraoperative blood loss and quick recovery in elderly patients with incarcerated sputum.It has unique advantages in detecting abdominal cavity,incarcerated sputum content and contralateral occultation.
