Risk Factors For Essential Hypertension Complicated With Chronic Kidney Disease In?80 Years Old Patients

Objective This study analyzed the clinical data of patients with essential hypertension(EH)over 65 years old in cardiovascular medicine,compared the incidence of chronic kidney disease(CKD)the <80-year-old EH patients(65-79 years old)with ?80-year-old patients EH patients(?80 years old),and analyzed for risk factors for CKD in 80 years of age or older EH patients.Methods A total of of 1555 patients with EH who were hospitalized in the Department of Cardiology,Second Affiliated Hospital of Nanjing Medical University,were enrolled in this study and divided into 2 groups according to whether they were ?80 years old: ?80-year-old group(n = 575)and <80-year-old group(n =980).Their general clinical data(sex,age,past history,medication history,admission blood pressure,admission diagnosis,etc.)and laboratory data(blood routine,urine routine,blood lipids,liver function,renal function,etc.)were collected and analyzed retrospectively.Data processing was performed by SPSS 22.0 software,and logistic regression analysis was used to analyze the risk factors of EH combined with CKD.Results (1)Compared with <80-year-old group,creatinine and uric acid were significantlyelevated in ?80-year-old group,and coronary heart disease,hyperuricemia,proteinuria,declined e GFR,and CKD were also more likely to happen(P<0.05).Diastolic blood pressure,fasting blood glucose,triglycerides,total cholesterol,low-density lipoprotein cholesterol,and e GFR were significantly reduced in ?80-year-old group(P<0.05).(2)In both groups,the incidence of CKD was significantly higher in man thanwomen,but the difference was not statistically significant(P>0.05).(3)Compared with <80-year-old group,?80-year-old group had significantlyhigher incidence of CKD(52.9% vs 29.5%,P<0.05).(4)In ?80-year-old group,the protective factor was high-density lipoproteincholesterol(OR= 0.516,95%CI 0.315-0.847;P = 0.009)and the risk factors for CKD were hyperuricemia(OR= 2.514,95%CI 1.750-3.611;P<0.001),age(OR = 1.072,95%CI 1.018-1.129;P = 0.009)and systolic blood pressure(OR = 1.019,95% CI 1.011-1.028;P< 0.001),.While,for <80-year-old group,the protective factor was high-density lipoprotein cholesterol(OR = 0.448,95% CI 0.278-0.722;P = 0.001),and the risk factors were hyperuricemia(OR= 2.729,95% CI 1.991-3.740;P < 0.001),diabetes mellitus(OR = 1.944,95% CI 1.420-2.662;P < 0.001),age(OR= 1.140,95%CI 1.101-1.182;P= 0.009)and systolic blood pressure(OR= 1.009,95%CI 1.003-1.016;P= 0.007).Conclusions(1)The incidence rate of CKD in ?80-year-old EH patients is obviously higherthan that in <80-year-old group.(2)The prevalence of CKD in ?80-year-old EH patients was not related to gender.(3)With the increase of systolic blood pressure,the prevalence of CKD in ?80-year-old EH patients increased.(4)The complication of CKD is associated with age,hyperuricemia,systolic bloodpressure and high-density lipoprotein cholesterol in ? 80-year-old g EHpatients.
