Objective:To analyze the risk factors for postoperative respiratory failure in patients with esophageal cancer,a retrospective analysis was conducted.Methods:493 patients were selected with esophageal cancer resection from January 2008to July 2017 in the Cancer Hospital of the General Hospital of Ningxia Medical University.Among them,a total of 329 patients were randomly selected,including 59 patients with respiratory failure(case group)and 270 patients without respiratory failure(control group).The general condition,preoperative,intraoperative,postoperative and tumor related indexes of the patients in the case group and the control group were statistically analyzed.The inclusion indicators were analyzed by single factor analysis(t test/x~2 test).In the univariate analysis,the index of P<0.15 continued to be included in multivariate logistic regression analysis.Moreover,statistically significant with P<0.05.Results:Univariate analysis showed that there were statistical differences in the age,body mass index,Pulmonary function grading,FEV1%FVC,smoking index,diabetes,heart disease,white blood cells(WBC),neutrophils(NEUT#),preoperative respiratory disease,operation time,postoperative pulmonary infection,anastomotic site and TNM staging between the casa group and control group(P<0.05).Logistic regression analysis showed that age,pulmonary function grading,diabetes,coronary heart disease,postoperative pulmonary infection,NEUT#and TNM staging were independent risk factors for postoperative respiratory failure in esophageal cancer(P<0.05).Among them,TNM staging is a protective factor.Conclusion:We conclude that patients with advanced age,poor lung function,preoperative diabetes,coronary heart disease,respiratory disease,longer operation time and postoperative pulmonary infection,are more likely to develop respiratory failure.Improving lung function and controlling blood glucose before surgery,Prevention of coronary heart disease,reduction of intraoperative injury,and postoperative active anti-infective treatment are beneficial to prevent postoperative respiratory failure.
