Objective To confirm the PiCCO monitoring technology in the early rehydration of elderly patients with severe burns and the prevention of complications during resuscitation.Methods The clinical medical data of 40 cases of elderly patients with severe burn admitted by the Burn Department of Gansu Provincial People's Hospital between Oct.,2015and Oct.,2018 were retrospectively analyzed,the cause of injury in all cases were flare burn,they were divided into two groups,namely,non-PiCCO monitoring group?i.e.traditional monitoring group?with 20 cases and PiCCO monitoring group?20 cases?on the basis of different monitoring means.Patients'data such as age,weight,burn area and post-injury hospitalization duration were collected.The actual total fluid infusion volume in the first one-three days after injured were gathered,the measured values of blood lactic acid,NT-proBNP and cTnT in 8 time-phase from the moment of admitting to the 7th day of hospitalization were collected.T-test and x2-test were conducted on the data,and the therapeutic effects of fluid infusion guided by two monitoring methods in elderly patients with severe burn were analyzed.Results 1.The differences of actual fluid infusion between two groups in the first two24hr had statistical difference?P<0.05?,while the difference in the third 24 hr had no statistical significance?P>0.05?.2.The differences of cLac measured on the 1st,2nd,3rd,5th,6th and 7th day in patients of both groups had statistical significance?P<0.05?,while the difference of cLac measured on the 4th day had no statistical significance?P>0.05?.3.The differences of cTnT measured on the 1st,2nd,3rd,5th and 7th day had statistical significance?P<0.05?,while the differences measured on the 4th and 6th day had no statistical significance?P>0.05?.4.The differences of NT-proBNP measured on the 1st,2nd,3rd,5th,6th and 7th day in both groups of patients had statistical significance?P<0.05?,and there was no statistical significance noted in the difference of NT-proBNP measured on the 4th day?P>0.05?.5.The cLac,NT-proBNP and cTnT values of the first 24 hours after injury in the two groups were immediately lower than those admitted to the hospital.The difference was statistically significant.Conclusion 1.PiCCO monitoring technology can scientifically and reasonably guide the early rehydration of elderly patients with severe burn;2.PiCCO monitoring technology can effectively reduce the occurrence of myocardial damage during fluid shock in elderly burn patients;3.PiCCO monitoring technology guide reasonable fluid replacement to avoid accumulation of blood lactic acid and reduce tissue hypoxia.
