The Application Of Levosimendan In Elderly Severe Heart Failure Under PICCO Monitoring

Objective: To investigate the clinical effect,safety and prognosis of levosimendan in the treatment of elderly severe heart failure under PICCO monitoring.Methods:A total of 77 patients with severe heart failure(heart function at grade III-IV)and aged ?60 years were enrolled in the CCU of the People's Hospital of the Autonomous Region.All of them were placed in PICCO.According to different treatment methods,36 patients in the experimental group(standard treatment+Levosimendan group)and 41 patients in the control group(standard treatment).For comparison between the two groups differences before and after treatment blood routine,biochemical indexes,PICCO parameters,BNP,output volume in 24 hours(synchronously),hospital days and cardiovascular end-point events were collected.Results:CO,GEF and dPmax increased after treatment in the experimental group compared with that before treatment,ITBV,EVLW and SVRI decreased after treatment in the experimental group compared with that before treatment,there were differences from the control group(P < 0.05),changes of before and after treatment in the two groups were different(P < 0.05).There were no significant difference in before and after changes between the two groups in PVPI and CVP treatment(P >0.05).BNP,hemoglobin,systolic blood pressure,and creatine kinase isoenzymes in the experimental group were lower than before treatment,there were differences from the control group(P < 0.05,changes of before and after treatment in the two groups were different(P < 0.05).There were no significant differences in liver and kidney functions and potassium between the two groups changes of before and after treatment,and no significant differences with the control group after treatment.Hospital days in the experimental group was lower than control group(P<0.05).Hospital mortality was no significant difference in two groups(P>0.05).Conclusion:Leosimendan can improve the hemodynamic disorder and cardiac function in elderly patients with severe heart failure,reduce the hospital days in CCU,delay the disease,and improve prognosis.Levosimendan may have no significant effect in liver and kidney functions in elderly patients with severe heart failure.Levosimendan may have an effect on blood pressure and hemoglobin.
