| ObjectiveHeadache symptoms after surgery under general anesthesia are including acute headache and chronic headache,and belongs to secondary headaches.Referring to the latest international headache classification launched by the International Headache Society(HIS)in 2018,postoperative headache after general anesthesia refers to patients with a history of headache or headless before surgery,are related to the surgical methods,types of anesthesia and the patient’s own factors.Postoperative headache after elective laparoscopic surgery under general anesthesia is a common complication after surgery,which is often associated with nausea,vomiting,eyes pain,visual impairment and other postoperative complications.Lighter without special treatment,and severe cases are warning signals and clinical manifestations of some fatal complications.However,the related factors of postoperative headache after elective laparoscopic surgery under general anesthesia are unknown Therefore,in this study,we research the related factors of postoperative headache in patients of elective laparoscopic surgery under general anesthesia.MethodsA total of 540 patients were included in the study after the approval of medical ethics.From the preoperative visit and screening of patients,the selected eligible patients were included in the study and intraoperative observation and relevant data were recorded.Patients were observed and followed up from the time of sober tracheal catheter removal until the third day after surgery,and whether or not the occurrence of headache after laparoscopic surgery under general anesthesia was recorded.We observed and recorded the patient’s general statistical data,lifestyle,methods of general anesthesia,types of laparoscopic surgery,anesthesia drugs use,intraoperative adverse reactions,and postoperative analgesic pump use,etc.Multivariate logistic regression analysis was used to identify the risk factors for postoperative headache in all patients for elective laparoscopic surgery under general anesthesia and patients without preoperative headache.ResultsThe incidence of postoperative headache in patients for elective laparoscopic surgery undergoing general anesthesia was 24.0%(n = 118).Among the patients with preoperative headache history(n = 78),79.5%(n = 62)had postoperative headache,and the incidence of postoperative headache in patients without preoperative headache history was 13.5%(n = 56).Risk factors related to the occurrence of headache after elective laparoscopic surgery under general anesthesia were gender(female)(P < 0.001;OR = 14.58),age(mainly 45-59 years)(P = 0.002;OR = 13.05),preoperative headache history(P < 0.001;OR = 13.81)sevoflurane(P < 0.001;OR = 8.06),intraoperative hypotension(P = 0.040;OR = 2.46),and surgical position(mainly Trendelenburg position)(P < 0.001,OR = 6.97).Among the patients without preoperative headache,the risk factors associated with higher incidence of headache after elective laparoscopic surgery under general anesthesia were gender(female)(P < 0.001;OR = 14.02),age(mainly 45-59 years old)(P = 0.010;OR =8.16),sevoflurane(P = 0.001;OR = 6.34),intraoperative hypotension(P = 0.020;OR = 3.23).Conclusionsgender(female),age(45-59 years),preoperative headache history,Sevoflurane,intraoperative hypotension,and surgical position(Trendelenburg position)are risk factors associated with postoperative headache occurrence after elective laparoscopic surgery under general anesthesia.In patients with no preoperative history of headache,gender(female),age(45-59 years),sevoflurane,and intraoperative hypotension were risk factors of postoperative headache occurrence after elective laparoscopic surgery under general anesthesia. |
