PartⅡ-1 THE EFFECTS OF CO2 ON RESPIRATROY AND CIRCULATROY FUNCTION OF PATIRNTS WITH GYNECOLOGICAL TUMOR IN LAPAROSCOPIC SURGERYObjective To investigate the effects of CO2 on respiratory and circulatory function of patients with gynecological tumor in laparoscopic surgery Method: The 256 patients were randomly divided into two groups,laparoscopic group (LO,128cases)and open group(AH,128cases)undergoing gynecological tumor. We monitored and recorded heart rates(HR),systolic blood pressure(SBP), diastolic blood pressure(DBP),mean arterial pressure(MAP),saturation pulse O2(SPO2),End-tidal CO2 pressure(PETCO2)and airway pressure(Paw)at respectively before(T0)and after pneumoperitoneum(PP)(laparotomy) 10min(T1),30min(T2),60min(T3),90min(T4).Results:SBP,DBP,MAP,Paw, PETCO2 were increased significantly after PP than those before PP in the group LO(P<0.05).SBP,DBP,MAP,Paw at after open were same as those of before open in the group AH(P>0.05).PETCO2 at after open were decreased significantly than those before open in the AH group(P<0.05).SpO2 weren't changed in two groups.Conclusion:The hemodynamics changed and theventilate function decreased during pneumoperitoneum patients.The patients with serious circulatory,respiratory complications underwent less laparoscopic surgery. PartⅡ-2 THE EVIDENCE-BASED EVALUATION TO THE EFFECTS OF CO2 ON SAFETY OF ANESTHESIA WAYS IN LAPAROSCOPIC SURGERYObjective:To evaluate the effects of CO2 on safety of laparoscopic anesthesia ways.Methods:We searched for randomized trials(published form Jan.1997 toDec.2007)in Anesthesiology that compared adjuvant general anesthesia or/and epidural anesthesia alone through CBMdisc,Medline,and Cochrane Anesthesiology Group's Specialised Register Central.Fifteen domestic public documents were choose into analysis.Results for safety were combined with fixed and random effects models and between-study heterogeneity was estimated.RevMan5.0 software was applied to process data. Results:In fifteen studies,the vital signs of general anesthesia demonstrated no statistically significant compared with epidural anesthesia(WMD>0,p= 0.11~0.80>0.05).Conclusion:This meta-analysis confirms that the Safe and reliable of general anesthesia same as epidural anesthesia in laparoscopic surgery. PartⅡ-3 EFFECTS OF CO2 ON IMMUNE FUNCTION OF PATIENTS IN LAPAROSCOPIC HYSTERETOMYObjective:To study the effects of laparoscopic hysterectomy and abdominal hysterectomy on immune system.Methods:A total of 20 patients with uterimyoma received laparoscopy hystere- ctomy(LH),20 patients were through abdominal approach(AH).The tumor necrosis factor-a(TNF-a),T lymphocyte subpopulation were measured 24 h before and 24 h,72 h after operation.Results:The concentrations of TNF-a in both groups increased significantly after operation(P<0.01),and they were significantly lower in LH group than that of AH group(P<0.01).Postoperative levels of CD3,CD4 and CD8 in AH group decreased significantly(P<0.01)whereas those of LH group decreased without statistical significance.Conclusion:Laparoscopic hysterec-tomy does not significantly affect immune function compared with AH suggesting that immunity is pre-served in the postoperative period. PartⅢ-1 THE EFFECTS OF CO2 PNEUMOPERITONEUM TO BIOLOGICAL BEHAVIOR OF CERVICAL CANCER CELL IN VITROObjective:To investigate the effects of CO2 pneumoperitoneum on growth, infiltration and metastasis of cervical cancer cell in vitro.Methods:With an artificial pneumoperitoneum model in vitrowere cervical cancer cells exposed to CO2 insufflation of different pressure and different time,then we determine the cell growth curve,cell cycle and the adhesive,invasive ability.To observe the change of growth,invasion and metastasis of cancer cell.Results:Proliferation after being exposed to CO2 pneumoperitoneum can be inhibit and then accelerate,but is not related to the pressure of CO2.the invasive,adhesive and invasive ability of cervical cancer cells after being exposed to CO2 pneumoperitoneum decrease notably.Conclusion:CO2 pneumoperitoneum can inhibit and then accelerate the proliferation,and inhibit the invasive,adhesive and invasive ability of cervical cancer cell. PartⅢ-2 The EffECTS OF CO2 TO CERVICAL CANCER CALL OF GROWTH AND METASTASIS IN VIVOObjective:To investigate the effects of CO2 pneumoperitoneum on growth and metastasis of cervical cancer cell in Vivo.Methods:To establish the nude mouse model of cervical cancer cell for CO2 pneumoperitoneum.The mice were randomly divided into three groups(CO2 pneumoperitoneum group,open abdomen group and Control group).Execute all the mice after four weeks,open the abdomen,search the organ in abdomen,disassociate the tumor tissue. Counting singulorum organ metastasis numbers,weigh metastasis tumor,detect the mice body weight degression after operation.Results:the tumor weight increased obviously in CO2 pneumoperitoneum group compare to open abdomen group and Control group.It had no predominance change in open abdomen group and Control group of tumor weight.The metastasis tumors were bigger on abdominal wall cut and more serious of growth and disseminateion in CO2 pneumoperitoneum group.The mice weight descenting in open abdomen group was larger than CO2 pneumoperitoneum group and Control group.Conclusion: CO2 pneu-moperitoneum can promote the growth and disseminateion of abdominal tumor than open abdomen operation. PartⅢ-3 EFFECT OF CO2 ON ANTI-ANOIKIS OF UTERINE CERVIX CANCER CELL LINEObjective:To investigate the effect of CO2 on the proliferation and anoikisres resistance of uterine cervix cancer hela cell line in vitro.Methods: MTT,suspension culture system,flow cytometry(FCM),clone formation test were used on the test group(hela cell by Exposing to CO2,8mmHg,4h)and the control group(hela cell not to CO2)to detect the ability of anoikis resistance of Uterine Cervix Cancer hela Cell line and the effect of CO2 on the proliferation and anoikis resistance of hela cell line.Pariedχ2 test was used for statistical analysis with SPSS13.0 softwae package.Results:Both the test group and the control group had the ability of anoikis resistance.The ability of anoikis in the test group was decreased than the control group(P<0.05).But CO2 could improve the clone formation and proliferation o f Hela cell line(P<0.05). Conclusion:CO2 could improve the proliferation and the growth of Uterine Cervix Cancer hela Cell Line.But CO2 cuold not improve the ability of anoikis resistance of Cancer Cell.Then,CO2 can not improve the invasion and metasta-sis of cancer cell. PartⅢ-4DIFFERENTIAL GENE EXPRESSION OF UTERINE CERVIX CANCER CELL INDUCED BY EXPOSING TO CO2Objective:To study the mechanism behind the biological behave changes induce by CO2 treatment.Methods:Hela cells were treated with CO2 and colonized,two clones which were highly tumorgenic were screened out.A double-channal microarray including 480 human tumor related genes were used to evaluate the gene expression profile changes after treatment.Results:Expression levels of 29 genes were changed in CO2A group,1 genes were upregulated with increased expression more than two times;28 genes were downregulated with their expression levels decreased lower than half.138 genes were changed in CO2B group,including 8 upregulated and 129 downregulated.The upregulated genes belonged to different pathways,including apoptosis,cell cycle,cell proliferation and signal transduction.The downregulated genes belonged to pathways including cell signaling,tumor suppressor genes,cell adhesion,MARK signal transduction and cell regulation.Conclusion:A different expression profiles induce by CO2 results in alternation of phenotype,increase cell proliferations. PartⅢ-5 THE EEFFECT OF CO2 ON EXPRESSION OFCAV1 AND CDK9 mRNA OF CERVICAL CANCER CELLSObjective:To investigate the diffence expression of CAV1 and CDK9 mRNA of cervical cancer cells with being exposed by CO2 with difference time and pressure.Methods:The real-time PCR was used to detect the diffence expression of CAV1 and CDK9 mRNA of cervical cancer before and after CO2 acting with difference time and pressure,and the plasmid DNA of CAV1 and CDK9 were constructed as control.Results:The expression content of CAV1 and CDK9 in hela cells after acting of CO2 was lower than that before acting of CO2,as well as the exprssion of CAV1 and CDK9 of hela cell in groupB(acting of CO2 with 8mmHg,and cells being incubated for 120 h)was also lower than that in groupA(acting of CO2 with 8mmHg,and cells being incubated for 24 h) and in control groups.Conclusion:The exprssion content of CAV1-mRNA, CDK9-mRNA could be detected by real-time PCR correctly.The results on expression of CAV1-mRNA,CDK9-mRNA decected by real-time PCR was coincide with that detected by gene-biochips.It was suggested that the expression of CAV1-mRNA,CDK9-mRNA could be down-regulated by CO2...
