Objective:To analyze the Clavien-Dindo classification of complications after laparoscopy assisted radical resection of colorectal cancer,and to explore the risk factors for postoperative complications,To evaluate the risk factors and put forward effective prevention measures to provide reference for the control of the incidence of postoperative complications after laparoscopic colorectal cancer surgery.Method:The clinical and pathological data from December 2012 to December 2017 review of the Department of general surgery of Affiliated Hospital of Tai’an Medical College underwent laparoscopic colorectal surgery in 232 patients,statistics of gender and age of patients,BMI and other basic information;collection of preoperative patients with concomitant diseases such as lung disease,diabetes,heart disease,hypertension and so on;collect postoperative complications: anastomotic fistula,intestinal obstruction urinary infection,pelvic infection,pulmonary infection,urinary retention,incisional hernia,deep vein thrombosis,hypoproteinemia,postoperative complications.According to the occurrence of postoperative complications,Clavinen-Dindo classification was applied to classify the complications.Statistical analysis software SPSS22.0 was used for univariate analysis,the 2 test was used,and Logistic regression analysis was used for multivariate analysis.A retrospective cohort study was conducted to investigate the risk factors of complications after radical resection for colorectal cancer,so as to find out the relative risk factors of complications,so as to avoid and reduce the incidence of postoperative complications effectively.Result:1.a total of 232 cases,male 131 cases,female 101 cases,age 24~88(57.73= & 13.1);2.The operation time was 180min(50 to 456min),and the amount of bleeding during the operation was 50ml(10~2000m1).3.A total of 47 patients had postoperative complications,the incidence of which was20.2%.According to the Clavien Dindo grading standard,the I level is 2.5%(6/232),the II level is 11.6%(27/232),the III-level is 2.1%(5/232),the III-b level is 1.7%(4/232),the IV-level a is 1.2%(III-b),and the fourth is,there is no postoperative complication.4.the statistical results show that: Logistic regression analysis showed that preoperative concomitant lung disease(OR=1.16,95%CI 0.07 ~ 1.81,P < 0.01),BMI(OR=2.57,95%CI 0.24 ~ 7.53,P=0.03),diabetes mellitus(OR=0.07,95%CI 0.03 ~ 1.37,P < 0.01)is an independent risk factor for complications of laparoscopic colorectal cancer root treatment of postoperative.Conclusion:1.In this paper,232 cases of laparoscopic radical resection of colorectal cancer were collected,and the complications were classified by Clavien-Dindo classification system.The results showed that the postoperative complications were mostly secondary complications.2.The results show that preoperative lung disease,diabetes and BMI are independent risk factors for postoperative complications of laparoscopic colorectal cancer.Therefore,in the course of clinical treatment,it is necessary to make clear the seriousness of the 3 risk factors.We should do a good job of preoperative assessment of the disease,and make effective clinical intervention for the pulmonary diseases before the operation so that the patients can tolerate the operation and strictly grasp the contraindication of the operation;the perioperative period should be actively controlled by the blood sugar and control the blood of the patients.The fluctuation level of sugar can reduce the incidence of postoperative complications and achieve good clinical results.
