Objective:To study the secretion characteristics and correlation of anti-Mullerian hormone(AMH)in patients with polycystic ovary syndrome(PCOS)with different clinical characteristics,and to provide a theoretical basis for the occurrence and development of PCOS,as well as a reference value for the disease evaluation and clinical diagnosis.Method:Patients who were amitted by the Gynecology Clinic or reproductive clinic of Yanan University Affiliated Hospital were selected as subjects from July 2017 to December 2018.According to the diagnostic criteria of Rotterdam(Rotterdam)in 2003,158 PCOS patients with sexual history were diagnosed.They were grouped according to four different clinical characteristics,including their menstrual cycle length,body mass index,total testosterone level,insulin resistance and non-insulin resistance.The differences of AMH levels and other indicators were compared among the four groups.At the same time,96 women with normal ovarian function were selected as the control group,such as due to oviduct or male and other factors in the reproductive department for assisted pregnancy treatment,healthy volunteers and pre-pregnancy physical examination.Statistical software SPSS 22.0 was used for statistical analysis of all data.Result: 1.Compared with the control group,serum AMH,follicle stimulating hormone(FSH),luteinizing hormone(LH),LH/FSH,estradiol(E2),testosterone(T),Homeostatic Model Assessment for Insulin Resistance(HOMA-IR)and body mass index(BMI)had statistical differences in PCOS group(P<0.05).Serum AMH level in PCOS group was significantly higher than that in control group.2.The diagnostic value of serum AMH for PCOS was evaluated by drawing ROC curve.The best diagnostic defining value of serum AMH for PCOS was 5.15ng/ml,the diagnostic sensitivity was 77.8%,the specificity was 88.5%,and the area under the curve was 0.863(P<0.05).3.There were significant differences in AMH,BMI,LH,LH/FSH,E2 and HOMA-IR between the two groups with different menstrual cycles(P < 0.05),but no significant differences in age,FSH and T(P > 0.05).4.There were significant differences in AMH,LH,T and LH/FSH among three groups of PCOS patients with different body mass index(P<0.05),but no significant differences in age,FSH,E2 and HOMA-IR(P>0.05).In comparison with AMH in different BMI groups,there was no significant difference in AMH between BMI 24-28 group and BMI(?28 group)(P>0.05);there were significant differences in the pairwise comparisons of remaining groups(P<0.05).5.There were significant differences in AMH,BMI,FSH,LH,LH/FSH,E2 and T between insulin resistance group and non-insulin resistance group(P<0.05);the AMH level in insulin resistance group was significantly higher than that in non-insulin resistance group.6.The levels of AMH,BMI,FSH,LH,LH / FSH,E 2 and HOMA-IR in PCOS patients with different levels of total testosterone were significantly different(P < 0.05);the levels of AMH in group T?2.44 nmol/L were significantly higher than those in group T<2.44 nmol/L.7.Pearson correlation analysis showed that AMH was slight positively correlated with HOMA-IR,BMI and T(r = 0.316,P = 0.000;r = 0.272,P = 0.001;r = 0.355,P = 0.000);Spearman correlation analysis showed that AMH was slight positively correlated with the length of different menstrual cycles(r = 0.386,P = 0.000).Conclusion: 1.Compared with women of normal reproductive age,serum AMH was significantly increased in PCOS patients,and it was closely related to the length of menstrual cycle,BMI,HOMA-IR and T in PCOS patients,which provided a basis for the study of etiology and pathogenesis.2.When serum AMH > 5.15 ng/ml,it has high specificity and sensitivity in the diagnosis of PCOS,it can provide reference for clinical diagnosis.3.AMH is positively correlated with the length of menstrual cycle,BMI,HOMA-IR and T.AMH is helpful to judge the severity of PCOS.
