Objectives1.Using the "OC-SENSOR Automatic Fecal Occult Blood Analyzer" to Perform fecal detection on the subjects and com Pare the immunocolloidal gold method to ex Plore the clinical significance of automatic quantitative fecal occult blood detection in the diagnosis of colorectal diseases.2.Using the "Human SDC2 Gene Methylation Detection Kit(Changan Heart)" to conduct stool detection in high-risk Po Pulation of colorectal cancer,and com Pare with quantitative fecal occult blood test,to ex Plore the exact Practical value of fecal SDC2 gene methylation in the diagnosis of colorectal cancer.Methods1.Select 650 in Patients who intend to com Plete colonosco Py in the Peo Ple’s Hos Pital of Ningxia Hui Autonomous Region from August 2019 to August 2020 as the research objects.Before the subjects underwent colonosco Py,fecal s Pecimens were collected for detection of fecal occult blood colloidal gold method and quantitative fecal occult blood test.Combined with the results of colonosco Py,the two methods of fecal occult blood were com Pared and analyzed for colorectal cancer,advanced adenoma,etc.Related statistical indicators such as the Positive detection rate,sensitivity,and consistency of the disease.2.Select 30 cases of colorectal cancer high-risk Po Pulation who intend to com Plete colonosco Py in the Peo Ple’s Hos Pital of Ningxia Hui Autonomous Region from Se Ptember 2020 to December 2020 as the research objects.Before the subjects underwent colonosco Py,stool sam Ples were collected for SDC2 gene methylation detection and quantitative fecal occult blood detection.Combined with the results of colonosco Py,the diagnostic efficacy of the two detection methods for colorectal cancer and advanced adenoma was com Pared and analyzed.Results1.The Positive rates of colloidal gold method and quantitative method were21.07% and 28.60%,res Pectively(P <0.05),among the 598 subjects included in the study.2.The sensitivity of quantitative method for colorectal cancer and advanced adenoma was 86.54% and 80.65%,which was significantly higher than that of colloidal gold method(55.77% and 54.84%),(P <0.05).The s Pecificity of quantitative method and colloidal gold method were 80.92% and 82.23%,res Pectively(P >0.05).The s Pecificity of quantitative method and colloidal gold method for the detection of advanced adenoma were 84.25% and 80.77%,res Pectively(P <0.05).3.There was no significant difference in the detection rate of lesion site between colorectal cancer and advanced adenoma by quantitative method(P >0.05).With the develo Pment of Pathological stage of CRC,the value of quantitative method showed an increasing trend,but there was no correlation between quantitative method and Pathological stage of CRC(P >0.05).4.Among the 290 cases of colorectal Poly Ps detected,there were 31 cases of advanced adenoma,147 cases of non-advanced adenoma,and 112 cases of non-adenomatous Poly Ps.The Positive rate of the quantitative method for advanced adenoma was 80.65%,significantly higher than 19.73% for non-advanced adenoma and8.93% for non-adenomatous Poly Ps(P <0.001).Among them,there was a statistical difference between the Positive rate of non-advanced adenoma and non-adenomatous Poly P(P <0.05).The Positive rates of Poly Ps with diameter ≥ 1cm and with diameter <1cm by quantitative method were 45.45% and 9.95%,res Pectively(P <0.001).The detection rates of single Poly Ps and multi Ple Poly Ps by quantitative method were17.14% and 13.64%(P >0.05).5.The sensitivity of SDC2 methylation detection in colorectal cancer and advanced adenoma was 100% and 17.65%,res Pectively,which was significantly higher than that of quantitative method(40% and 11.76%,P <0.05).The Positive rate of SDC2 gene methylation combined with quantitative assay for colorectal cancer and advanced adenoma was 45.45%,which was higher than that of quantitative assay(18.18%)and SDC2 gene methylation(36.36%)alone(P <0.05).6.There were no significant differences in the methylation status between SDC2 gene and Patients with colorectal cancer and advanced adenoma in different ages(<55years old and ≥55 years old),gender(male or female),lesion site(Proximal or distal),lesion size(≤2cm and > 2cm)(P >0.05).Conclusions1.Quantitative fecal occult blood test is significantly better than immunocolloidal gold method in identifying colorectal cancer,advanced adenoma and colorectal Poly P.There was no correlation between quantitative detection and the site of colorectal cancer and Precancerous lesions,and Pathological stage,while the detection value was Positively correlated with tumor diameter.The detection rate of colorectal Poly Ps by quantitative method is related to the nature and size of Poly Ps,but not to the number of Poly Ps.2.Fecal SDC2 gene methylation is highly sensitive to the diagnosis of colorectal tumors,and has obvious advantages com Pared with quantitative stool occult blood detection;The combined detection of SDC2 methylation and quantitative method is better than single detection in the detection of colorectal tumors.There was no significant correlation between the abnormal methylation of SDC2 gene in feces and CRC and the age,gender,lesion site and lesion size of the Patients with advanced adenoma.
