ObjectiveTo develop a scale of Catheter-associated Urinary Tract Infection(CAUTI)knowledge-attitude-practice(KAP)and to test its psychometric Properties, and to provide effective assessment tools for clinical evaluation of knowledge, attitude and practice of CAUTI prevention.MethodsBased on KAP theory and CAUTI preventive current guideline, the scale was developed by using literature review,Delphi expert inquiry.A questionnaire survey was conducted among 300 medical workers in a third grade A hospital in Qingdao.Exploratory factor analysis and confirmatory factor analysis were both conducted to screening the items further. The psychometric properties of scale was tested.ResultsPart1:the finalized scale included 4 subscales and 45 items, amount to 63.21%of the variance contribution rate. The total Cronbach's alpha was 0.914,and the split half reliability was 0.812.The-Level CVI scale horizontal content validity index is 0.895,and the I-CVI item-level CVI item horizontal content validity index is 0.846~1.000.The correlation of item-tototal was from 0.711~0.862,and the correlation coefficient of item-to-item ranged from 0.425~0.564.Part2:for the analysis of the basic level before training, the influencing factors affecting the scores of the ICU Medical Staff CAUTI Cognitive Questionnaire include the age, working years, professional title, academic qualifications, position, rotation or training department, based on the knowledge and belief theory. The CAUTI prevention training program, during the 9-month implementation period, conducted a three-cycle training course intervention for ICU nurses. After the intervention, the nurses'knowledge scores were increased to different degrees(P<0.01),the catheter usage rate was reduced from76.80%to 56.34%(P<0.01).At the same time the incidence of CAUTI decreased from6.20/thousand catheter day of baseline survey result to 3.14 per thousand catheter day(incidence rate ratio=0.51;?~2=4.42 P<0.05).The incidence rate of CAUTI population decreased from 5.99/thousand catheter day of baseline survey result to 1.92/thousand catheter day(incidence rate ratio=0.32;?~2=4.84 P<0.05).ConclusionThe scale of CAUTI KAP has good reliability and validity. It can be used to evaluate the knowledge, attitudes and behaviors of CAUTI prevention. The training content developed by the knowledge and belief theory can effectively improve the knowledge and measurement of prevention of CAUTI in ICU nurses, thus effectively reduce the incidence of CAUTI in hospitals.
